Fort Collins. 3. Work Order Procedure: A. All job estimates must be submitted on a unit price basis consistent with the prices established in the Methods of Award/Bid Schedule section. Contractor will invoice for all jobs completed on a unit price basis with the prices established in the Method of Award/Bid Schedule section, including appropriate mark up on materials, if any. Material invoices must be included with the billing invoices. C. Work order number must be included on the billing invoices. 4. Hourly rates charged for work ordered under this agreement will be: A. COLLINS SAFETY PROGRAM Encompass Furnishes each employee a place of employment that is free from recognized hazards, causing, or likely to cause, death or serious physical harm. Encompass' comprehensive safety program is in compliance with the Occupational Safety and Health Act of 1970 and is revised when new requirements or laws warrant such revision. Encompass has a full time Environmental Safety and Health Manager to maintain our comprehensive safety program, and to facilitate compliance with the Occupational Safety and Health Act. This provides for frequent and regular inspections of our job sites, materials and equipment. Our safety department and field supervision is fully capable of developing site -specific safety plans and programs for non -routine tasks and projects insuring all critical aspects of safety are properly addressed. Training at Encompass is a priority. Encompass' accident prevention program consists of holding weekly safety meetings on the job site pertaining to site -specific issues, along with providing detailed safety instruction to our employees throughout their professional career. This training begins when they enter our company as an apprentice electrician and continues through their upward progression into management. Each Encompass manager and supervisor completes an OSHA 10-hour course of instruction that provides them with the necessary information to identify those hazards that are common in construction. This training provides them with the information they need to successfully implement our safety program on their projects. Each employee is provided with a field safety manual and a safety briefing at the time of employment. Our comprehensive safety manual provides technical guidance and support on the more difficult compliance issues and is periodically updated as new regulations warrant. Our program provides the tools for frequent and regular inspections of job sites, materials, and equipment. The company safety department, project managers, and job site foremen facilitate this rigorous inspection program. To be successful, our program embodies the proper attitude toward injury and illness prevention on the part of both supervisors and employees. We recognize that the responsibility for safety and health is shared at the different levels of our organization: • Encompass accepts the responsibility for leadership of the safety and health program, for its effectiveness and improvement, and for providing the safeguards required to ensure safe conditions. • All levels of the organizational structure are responsible for developing the proper procedures related to safety and health for themselves and in those they supervise. COLLINS This requires all operations to be performed with utmost regard for the safety and health of all personnel involved, including them. • Employees are responsible for their genuine cooperation with all aspects of the safety and health program, including compliance with all rules and regulations, and for continuously practicing safety while performing their jobs. In accomplishing these goals and insuring compliance with these responsibilities, Encompass practices behavioral based safety and modern safety management principles, which provides our company with a way of reducing lost -time injuries, through the analysis of the work environment, and the implementation of proven performance management methods. These behavioral safety principles have a dramatic and positive effect resulting in the reduction of the number of lost -time and minor injuries, along with their related severity. This is clearly demonstrated in the low and consistent numbers affecting our company's experience modification rate: 1994 -1.02 1 1995 - .94 11996 - .84 1 1997 - .62 1 1998 - .79 1 1999 - .78 12000 - .79 SAFETY INCENTIVE PROGRAM Safety incentives begin immediately upon starting employment at Encompass and include foremen, journeymen, and apprentices. This program is a token program that rewards employees for each month they work injury and accident free. The tokens are exchangeable on a quarterly basis for valuable gifts, such as: stereo, audio and video components, exercise and sporting equipment, personal and professional tools, hunting and camping equipment, and home appliances. SAFETY DISCIPLINARY PROCEDURES Employees who fail to comply with Encompass safety rules and OSHA safety policies are subject to disciplinary action, up to and including termination, depending upon the seriousness of the offense and the employee's work record. Management will determine the degree of discipline imposed. Discipline can and will include any of the following: • Verbal or written warning • Written warning with time off (without pay) or possible termination • Termination All safety violations incurred by the employee will be kept in the employee's personnel file. CITY OF FT. COLLINS WORKPLACE DRUG/ALCOHOL POLICY The policy of Encompass has always been that illegal drugs or alcohol have no place on the job. Encompass has always been proud of the quality of work that we have provided to our customers and the level of safety we have maintained throughout all areas of our business. Encompass is seriously concerned about the adverse effects of alcohol and drug abuse on the health and safety of our employees. All employees are made aware that the unlawful manufacture, distribution, dispensing, possession or use of any illegal drugs, controlled substance, or alcohol would threaten the quality of work, health, and safety standards which we are committed to uphold. As a result, Encompass has adopted a formal drug and alcohol policy prohibiting the unlawful manufacture, distribution, dispensing, possession, and use of any illegal drugs, controlled substances, or alcohol by employees of Encompass in the workplace. Reporting to work or working while impaired by alcohol or drugs is prohibited by company policy. The workplace includes all job sites and offices, including temporary field offices. This policy also extends to employees when operating company owned or leased vehicles and when on company business away from the normal workplace. Employees who are required to take prescribed drugs for medical reasons must notify supervisory personnel and advise them of any possible side effect. Encompass conducts mandatory pre -hire and post -accident drug testing. TQM/QUALITY CONTROL Total Quality Management CITY OF FT. COLLINS Encompass' Total Quality Management Program is eight (8) years old. We call it Encompass Top Quality. Our Top Quality Program uses Make Us Great (MUG) forms to receive improvement ideas from employees. Since inception, we have received over 1,400 MUG's and implemented the majority of the ideas. Many MUG's lead to mini -teams to develop the idea. One of the MUG's addressed our customer's needs on the job site. The objective is to determine the needs of our customers. The ultimate purpose of the Top Quality Program is to improve our processes and become more cost efficient. These efficiencies will ultimately benefit our customers.
