Service Provider agrees to perform the services identified above and on the attached forms in
accordance with the terms and conditions contained herein and in the Services Agreement
between the parties. In the event of a conflict between or ambiguity in the terms of the Services
Agreement and this work order (including the attached forms) the Services Agreement shall
control.

PROPOSAL TWO
ITEM #1 - RESURFACE OF THREE UPPER WEST COURTS AT ROLLAND MOORE PARK
Work to proceed as follows:
1. Remove nets and net posts
2. Remove all chain link
3. Remove necessary fence posts and frame work to allow access
4. Clean and fill all cracks with Vance Brothers Crackfill
5. Apply 12" wide Petro Tack over all cracks
6. Apply Petro Mat over entire area
7. Place and compact 2" layer of hot mixed asphalt
8. Re -install all fence posts, frame work and approximately 400 L.F. of chain
link
9. install approximately 360 L.F. of new 9 gauge tennis court fabric (all North &
South sides)
10. Flood and patch all areas holding more than 1/16" of water
11. Apply two coats of Hawker sanded resurfacer
12. Apply two coats of Hawker colorcoat concentrate
13. Paint white lines to meet USTA specifications
14. Re -install tennis net posts, nets and center straps
15. Courts to be dark green playing areas with medium green border
16. Total job site cleanup
