Sealed bids will be received and publicly opened at the office of the Director Of Purchasing and
Risk Management, 256 West Mountain Avenue, Fort Collins, Colorado 80521, at the time and
date noted on the bid proposal and/or contract documents
Bids must be received at the Purchasing Office prior to 3:OOp m. four clock), February 24,
2000.

Special Instructions
All bids must be properly signed by an authorized representative of the company with the legal
capacity to bind the company to the agreement Bids may be withdrawn up to the date and hour
set for closing Once bids have been accepted by the City and closing has occurred, failure to
enter into contract or honor the purchase order will be cause for removal of supplier's name
from the City of Fort Collins' bidders list for a period of twelve months from the date of the
opening The City may also pursue any remedies available at law or in equity Bid prices must
be held firm for a period of forty-five (45) days after bid openings
Submission of a bid is deemed as acceptance of all terms, conditions and specifications
contained in the City's specifications initially provided to the bidder Any proposed modification
must be accepted in writing by the City prior to award of the bid
Only bids properly received by the Purchasing Office will be accepted All bids should be clearly
identilied by the bid number and bid name contained in the bid proposal
No proposal will be accepted from, or any purchase order awarded, to any person, firm or
corporation in default on any obligation to the City
Bids must be furnished exclusive of any federal excise tax, wherever applicable

TRAFFIC CONTROL DEVICES
Section 614 of the Standard Specifications is hereby revised as follows
Subsection 614 15 shall be revised as follows
'I Ills work shall consist of furnishing, installing, moving maintaining and removing temporary traffic signs,
advance warning arrows panels, barricades, channehzing devices, and delineators as required by the latest
revisions of the "Manual on Uniform Traffic Control Dcvices loi Sticets and Highways" (MUI'CD), the
City of Fort Collins' "Work Area Traffic Control Handbook", and the "City of Fort Collins Design and
Construction Ci teria , Standards and Specifications for Streets, Sidewalks, Alleys and Other Public Ways",
July 1996
]n the event of a conflict between the MUICD criteria and the City's criteria the City's enters shall
govern
When a device is not in use, the Contractor shall remove it from the project for the period it is not needed
Devices temporarily not in use shall, as a minimum, be removed four the area Moving will include

Subsection 614 16 shall include the following
All traffic control devices placed for this project must meet or c\ceed the minimum standards set forth in
lire MUTCD All traffic control devices shall be clean and in good operating condition when delivered and
shall be maintained in that manner on a daily basis All traffic control devices shall be clearly marked and
free of crossed out information or any other form of defat ci rent that detracts from the purpose for which
they are intended (r e crossed out infra nwtion, information written in long -hand style etc )
Additionally, any sign blank with sign faces on both sick,, must have lire back sigh face covered when in
use to avoid confusion to motorists travehng in the opposite direction and other potentially affected parties,
such as residents affected by any information the sign may present
Subsection 614 20 shall be revised as follows

For all locations, a I'taffic Control Plan shall be pupaied The Traffic Control Plans shall be on City
supplied forms The Traffic Control Plans shall be submitted for approval to City Traffic Operations by
12 00 noon, two working days pilot to the conunenctinenl of work (Note Traffic Control Plans for work
done on Monday and Tuesday shall be submitted the previous Friday by 9 00 a or ) All plans shall be
delivered to City Traffic Operations, 625 Ninth Street Facsimiles of plans shall not be allowed No phase
of the construction shall start until the Traffic Control Plan has been approved Failure to have an approved
I iatfic Control Plan shall constitute cause for the City to stop work, as well as the Contractor's forfernuc of
payment for all work and materials at that location, with no adjustment in the contract time

(1) A detailed diagram which shows the location of all sign placements including advance
construction signs (if not previously approved) and speed limit signs, method, length and time
duration for lane closures, and location of flag persons
(2) A tabulation of all traffic control devices shown on the detailed diagram including, but not limited
to construction signs, vertical panel vertical panel with Tight type I Type II, and Type III
barricades cones, drum channehzing devices, advance warning flashing or sequencing arrow
panel Ccitam tiattic control devices may be used for more than one operation of phase

Parking RestnUron Plans shall be submitted and approved which show the location and quantity of "NO
PARKING" signs, the clan, to be placed, and the (late to be removed The plans shall be prepared on City
supplied forms The Parking Restriction Plans shall be submdted to City "traffic Operations by 12 00

Subsection 614 21 shall be revised as follows
Traffic Control Management shall be performed by a Tiaffie Control Supervisor (TCS) The TCS(s) shall
possess a current American liattic Safety Services Association (ATSSA) certification as a Worksite
Traffic Control Supervisor or Colorado Contractor's Association (CCA) certification as a Tiaffie Control
Supervisor (Proof of certification shall be presented to the City'liaffic Control Manager, and when
requested by a City representative, for each TCS utrlrzed on this project ) The TCS shall have a minimum
of one year experience as a ,citified I CS
The FCS shall be equipped with a cellular phone I he cost of this phone shall be incidental to the day or
hour pay inn for '1 CS
The ICS's duties shall include, but not be limited to
(1) Pr,pai mg, revising and submitting Traffic Control Plans as requited
(2) Duc,t supervision of project flaggers
(3) Coordinating all traffic control related operations, including those of the Subcontractors and
suppliers
(4) Coordinating project activities with appropriate police and fire control agencies, Transfort, school
districts and other affected agencies and parties prim to construction typed hand delivered

notification to all bits messes and residents at least 24 hours pilot to construction (The nonfication
of residents and businesses may be accomplished by a iepresentauve of the TCS )
(5) Maintaining a project traffic control diary which shall become port of the City's project records
(6) Inspecting traffic control devices on every calendar day tot the duration of tilt project
(7) Insuring that haf fie control devucs are functioning as requned
(S) Overseeing all tcquiremenis coveted by the plans and specifications which contribute to the
convenience safety and orderly movement of traffic
(9) Flagging
(10) Setting up ttaffrc control devices
Notification of residents and businesses ~hall be the responsibility of file TCS and shall consist of
distributing letters indicating the nature of the work to be completed, any special instructions to the
residents (i e limits on lawn watering during concrete pouring, etc), the dates and times of the work, and
the parking and access reshrctrons that will apply, as well as thorough information placed on "NO
PARKING" signs Sample letters may be provided by the City Represcntdhve, and shall be distributed
Pilot to the commencement of each phase of the Nvoik Approved letters shall be distributed a minimum of
24 hours prior to the commencement of work (Note The tune fiame criteria for distributing letters is the
same as for posting "NO PARKING" signs ) The cost for fit eparutg and distributing the letters shall be
included in the cost for 7 CS
Ttaffic control management shall be mmntaincd on a 24 hour pet day basis The Contractor shall make
arrangements so that the Iraffic Control Supervisor or his approved representative will be available on
evety winking day, "on call" at all tunes mid available upon the City's request at other than normal working
hour
The ICS's will be paid only tot days the Contractor works, and as duected by the Engineer On weekends
and other days the Contractor is not working, tht I CS rs dl not be paid Tune spent maintaining signs on
the weekends, holidays, bad weather days, and other days the Contractor does not work shall be
included in the unit prices for the equipment ['line sprit setting up equipment, modifying equipircnt,

The Traffic Control Supervisor shall have up to date copies of the City of Fort Collins' "Work Area Traffic
Control Handbook", and Part VI of the MUfCD, pertaining to traffic controls for street and highway

Subsection 614 23 shall be revised as follows

The City may elect where applicable, to award bids on an individual item/group basis or on a
total bid basis, whichever is most beneficial to the City The City reserves the right to accept or
reject any and all bids, and to waive any irregularities or informalities

in and sharing the expenses of their household, shall have a financial interest in the sale to the

Purchasing Office or the City Clerk's Office
Cement restrictions City of Fort Collins Resolution 91-121 requires that suppliers and producers
of cement or products containing cement to certify that the cement was not made in cement
kilns that burn hazardous waste as a fuel

WE HEREBY ENTER OUR BID FOR THE CITY OF FORT COLLINS' REQUIREMENTS
FOR MISCELLANEOUS CONCRETE PROJECT PER THE BID INVITATION AND ANY
REFERENCED SPECIFICATIONS
The Contractor must provide all equipment, labor and materials deemed necessary to remove,
haul, backfill, compact, install and patch concrete curb, gutter, sidewalk, pedestrian access
ramps, concrete pavement, aprons, crosspans, trails, and inlets for various City departments
Needed services during the contract period will be requested through issuance of work orders
There is no guaranteed minimum amount of services to be ordered No work order exceeding
$50,000 will be issued The City reserves the right to supply any or all materials This contract
is a one year contract beginning May 1, 2000, with 4 optional years to renew and shall be
administered by the Engineering Department, but may and will be utilized by other City
Departments
SCOPE OF WORK
GENERAL
A This bid will be for various tobsites, as required by City Departments and should
not be construed as one site Each tobsite will be based on isolated locations
which are separated by more than 1 city block Needed services during the
contract period will be requested through issuance of work orders No work order
exceeding $50,000 will be issued
B The Contractor must provide all labor, materials and equipment, including traffic
control and barricading to accomplish the work in accordance with City standard
specifications Replacement of curb, gutter and sidewalk shall match existing,
i e , vertical curb shall be replaced with vertical curb, etc , or as directed by
Engineer or Protect Manager
C Contractor must maintain a local telephone number, and must be able to be
reached by telephone and facsimile transmissions during prescribed business
hours (8 hours per each 24 hour working period - Monday through Friday)
Contractor will respond to all City non -emergency telephone transmissions in a
reasonable time (Generally meaning within 24 hours on the next working day)
2 COMPLIANCE STANDARDS

A Contractor must refer to City of Fort Collins Design and Construction Criteria,
Standards and Specifications for Streets, Sidewalks, Alleys and Other Public
Ways, July 1996, for this project These documents are available from the City
Engineering Department
B The Contractor must submit concrete mix designs for approval by the City
Engineering Department prior to starting work The City may require the
Contractor to use rapid set concrete
C The Contractor must be licensed with the City of Fort Collins and shall comply
with all applicable Federal, State and Local Laws and regulations for the duration
of the contract
D All a5phalt patching shall be performed in accordance with the current City
specifications and by a contractor licensed with the City of Fort Collins
SERVICE AGREEMENT AND INSURANCE
A Contractor must enter into the attached services agreement and provide the
required insurance (Exhibit "B") This agreement is effective for one year from
the date on the service agreement At the option of the City, the Agreement may
be extended for additional one year periods not to exceed four (4) additional one
year periods Pricing changes shall be negotiated by and agreed to by both
parties and may (unless approved in waiting) not exceed the Denver - Boulder
CPI-U as published by the Colorado State Planning and Budget Office Written
notice of renewal shall be provided to the Service Provider and mailed no later
than ninety (90) days prior to contract end
WORK ORDER PROCEDURE
A All job estimates must be submitted on a unit price basis consistent with the
prices established in the Bid Schedule section
B Contractor will invoice for all jobs completed on a unit price basis with the prices
established in the Bid Schedule section
C Work order number must be included on the billing invoices
D All jobs must begin within a ten (10) day period following issue of a work order
Each work order shall be signed and returned to the issuing department within 48
hours If the low responsive, responsible bidder cannot meet this performance
specification, the City has the right to offer the project to the next lowest
responsive, responsible bidder according to prices listed from their submitted bid
schedule Exceptions to this policy will be made for inclement weather
preventing a timely response by the Contractor (a one -day extension for each
day of bad weather will be given), or a previous understanding between the
Contractor and the City's departmental representative allowing more flexibility
with regard to the individual project It will be the Contractor's responsibility to
make these arrangements with the City's representative
PATCHING REQUIREMENT

A All concrete placed adjacent to asphalt pavement will require placement of a hot
mix asphalt patch, unless otherwise directed by the City Engineer's office The
minimum length shall be one (1) foot longer than the concrete replacement
Asphalt next to new crosspans and aprons shall be sawcut a minimum of 30
inches from the new concrete, and asphalt next to new curb and gutter shall be
sawcut a minimum of 24 inches from the new concrete so that the patching crew
can provide a clean, effective patch Patching limits shall be determined by the
City Engineer, or his representative, and shall meet current City of Fort Collins
patching standards for limits and depth The Contractor shall patch the asphalt
before removing the barricades and within seven (7) days of the placement of the
concrete curb and gutter If cold weather does not allow for a hot mix asphalt
patch, the Contractor shall place a temporary asphalt patch and maintain the
patch until a hot mix patch can be installed
6 INSPECTION
A It shall be the Contractor's responsibility to notify the City Engineer, or their
representative, that the work is ready for inspection Every request for inspection
shall be made at least twenty-four 24) hours before inspection is desired Any
work completed by the Contractor and not inspected and accepted by the City
engineer's office shall be removed and replaced at the Contractor's expense
7 AWARD
A Prices bid shall be based upon Project Specifications and shall be each Bidder's
responsibility to familiarize themselves with the complexities involved for each
type before entering a bid Payment will be processed by City Departments upon
the presentation of invoicing
B Award of this bid will be made in the best interest of the City Evaluations will be
completed using the estimated quantities given, times the unit price
8 TRAFFIC CONTROL

REVISION OF SECTION 104
TRAFFIC AND PARKING CONTROL
Section 104 of the Standard Spec i ications is hereby revised as follows
Subsection 104 04 shall include the Following
It shall be the Contractor's responsibility to clear parking tionm the sueets ti IlLIl such parking will intettere
with the work Prior to work that requires the, strect(s) or portion thereof, to be closed to parking and/m
traffic, the street(s) shall be posted for "NO PARKING" The placement of these signs shall lake place at
least 24 hours prior to the commencement of work and shall clearly show the day, date and times that the
message on the sign is in effect (For example, if a sheet is to be patched on Wednesday, July 2, the street
shall be posted no later than Tuesday, July 1, by 7 00 a in with a sign that leads similar to "NO PARKING,
WEDNESDAY, JULY 2, 7 00 A M TO 6 00 P M , PA PCIIING) "NO PARKING" signs shall remain in
place until the street is opened to traffic and all clean up opetatrons completed
All information on the "NO PARKING" signs, with the cxceptron of the date, shall be in block letters
permanently affixed to the sign Any infotmahon added to a sign, such as dates shall be clearly legible and
written in block style lettcts The "NO PARKING" signs shall be in effect for one or two days only, prior
to construction
Any work done by the Contractor without traffic control will not be paid for tinder the terms of this
Contract file Contractor will not be paid for tratfic control costs incuued dining Contractor caused
delays
At of near the end of each wotk clay a I ept esentative of the Contractor, the Traffic Control Supcivism, and
the City Reptesentattve will meet to drsenss the pwgiess of the wolf, and the placement of traffic control
devices including "NO PARKING" signs Any necessary idjnslmenls shall be made The Conttactot shall
also review at this time with the City Representative the proposed means of handling parking and traffic
control for upcoming well, It is the responsibility of the Contractor to minimize any utconveinence to the
public as a result of this wot k
At the above teferenced meeting, the quantity of tmatlic control devices used that day shall be agreed upon
by the Contractor and the City Representative "file Contractor shall utilize forms approved by the City for
the tabulation of fialfic Control Devices utilized
The Conhaclm shall maintain access at all lmmcs to all businesses within time project
Any changes in the uatfic control, including additional signs, barricades, and/or tlaggems needed m the field
shall be immediately implemented as directed by the City Pepresentalrve
The cost for traffic control is coveted in "Revision of Section 614, Traffic Control Devices" found herein
p
