The City of Fort Collins wishes to extend the agreement term for the above captioned proposal per
the existing terms and conditions as well as any change orders implemented during the course of
the contract The term will be extended for one (1) additional year, June 24, 2001 through June 23,
2002
If the renewal is acceptable to your firm, please sign this letter in the space provided and return it
to the City of Fort Collins, Purchasing Division, P O Box 580, Fort Collins, CO 85022, within the
next fifteen days We will also need updated insurance forms and any price changes you are
requesting

exceptions that the appropriate workers' compensation rate would be used.

FIXED MARK-UP PERCENTAGE
All clerical and administrative support positions will be billed at a fixed markup
percentage using the formula of thr pay rate plus fort' -three percent (43%)
of the pay rate. Non -clerical will be billed at a fixed markup percentage using
the formula of the pay rate plus forty-four & sixty-seven hundredths (44.67%)
of the pay rate.

department contacts and the comments were favorable for both Manpower and Express So, I would like
to extend their contracts for one more year I don't see any compelling reasons to go out to bid

INSURANCE REQUIREMENTS
1 The Service Provider will provide, from insurance companies acceptable to the City, the
insurance coverage designated hereinafter and pay all costs Before commencing work under
this bid, the Service Provider shall furnish the City with certificates of insurance showing the
type, amount, class of operations covered, effective dates and date of expiration of policies,
and containing substantially the following statement

"Should any of the above described policies be canceled before the expiration date
thereof, the issuing Company will mail 30 days written notice to the City of Fort
Collins."
In case of the breach of any provision of the Insurance Requirements, the City, at its
option, may take out and maintain, at the expense of the Service Provider, such insurance as
the City may deem proper and may deduct the cost of such insurance from any momes which
may be due or become due the Service Provider under this Agreement The City, its officers,
agents and employees shall be named as additional insureds on the Service Provider's
general liability and automobile liability insurance policies for any claims ansing out of the
negligent performance of work performed under this Agreement, except for the
negligence and/or willful misconduct of the City, its officers, agents and employees.
2 Insurance coverage shall be as follows
A Workers' Compensation & Employer's Liability The Service Provider shall maintain
during the life of this Agreement for all of the Service Provider's employees engaged in
work performed under this agreement
1 Worker' Compensation insurance with statutory hunts as required by Colorado
law
2 Employer's Liability insurance with liiruts of $100,000 per accident, $500,000
disease aggregate, and $100,00 disease each employee
B Commercial General & Velucle Liability The Service Provider shall maintain during
the life of this Agreement such commercial general liability and automobile liability
insurance as will provide coverage for damage claims of personal injury, including
accidental death, as well as for claims for property damage, which may anse directly or
indirectly from the negligent performance of work under this Agreement Coverage for
property damage shall be on a "broad form" basis The amount of insurance for each
coverage, Commercial General and Vehicle, shall not be less than $500,000 combined
