This project consists of the installation and configuration of up to a four (4) node cluster of Lefthand Networks Storage Modules to create an lP-SAN supporting centralized storage for various Windows, Linux and Netware Operating systems, as well as to migrate existing data on Lefthand NSM160 units to the newer DL320 platform. Lewan & Associates (Lewan) is a systems engineering firm and systems integrator with experience in deploying IT solutions for distributed computing environments. Lewan has experience across multiple platforms, operating systems, database management systems and networks. We have a proven history of a dedicated professional team to helping our clients implement high performance, cost effective and manageable client/server solutions. Our goal is to ensure that your technical infrastructure effectively supports the attainment of your business requirements and upcoming projects. In consideration of the receipt of this document, City of Fort Collins agrees not to reproduce or make this information available in any manner to persons outside City of Fort Collins directly responsible for evaluation of its contents. Project Overview The goal of this engagement is to assist City of Fort Collins with the following tasks: • Installation of up to four (4) Lefthand Networks IP-SAN modules, Lefthand Networks SAN Management console, and clustering of up to four (4) Lefthand Networks IP-SAN modules. • Installation and configuration of iSCSI drivers on up to 5 Microsoft Windows, Redhat Linux, Netware servers or any combination thereof. • Creation of up to three iSCSI volumes per server with a maximum of 15 volumes. • Presentation and formatting of up to three iSCSI volumes per server with a maximum of 15 volumes. • Data Migration of existing data located on NSM160 units to the new DL320 units. Notice: Advanced VLAN configurations are normally required for the IP SAN environment. VLAN configuration is not covered under this statement of work. Lewan can provide advanced VLAN assistance under a different statement of work if your staff requires assistance with these services. Lewan & Associates Lewan and Associates will perform the tasks under this SOW using a phased approach: Phase 1 - Project Kickoff Meeting Phase 2 - HardwarelSoftware Installation and Configuration Phase 3 - Configuration Deliverable Phase 4 - Validation and Testing Phase 5 - Project Closeout The tasks and deliverables associated with each phase are as follows: Phase 1 - Project Kickoff Meeting Lewan Project Manager or Consultant will lead a planning and preparation meeting with Customer contacts to determine roles and responsibilities and to finalize the schedule. During this meeting, Lewan and Associates will confirm Customer readiness with regard to Consultant access requirements, physical facility requirements, Customer configuration, and Customer's completion of the Pre - Engagement Checklist. Suggested participants are: Project Manager System Administrator Database Administrator Network Administrator Agenda Review architecture Review scope of implementation Review resource requirements Review Pre -Engagement Checklist Discuss system downtime requirements Review and agree on test plan Review Project Closeout process Phase 2 - Hardware/Software Installation and Configuration Tasks: • Verify software, hardware, and network requirements per Lefthand requirements • Discuss and recommend naming conventions • Configure Lefthand Network Storage Modules in rack, configure IP addresses, setup network teams • Install and configure Lefthand SAN/iQ Management Software • Install and configure iSCSI software on specified servers • Configure Management Group and Storage Clusters • Configure SAN Volumes and present volumes to servers • Format volumes and test data transfer • Data Migration of existing data located on NSM160 units to the new DL320 units. Lewan & Associates Phase 3 - Configuration Deliverable Tasks: Provide the customer with a "Post Installation Report" showing an overview of the installed environment and any modifications made during implementation and testing. Phase 4 - Validation and Testing Tasks: • Validate SAN volumes were successfully presented to customers servers and that data can be copied to the volumes • Validate all data was successfully migrated from NSM160 units to the new DL320 units. • Execute and validate that all tests were successfully completed per customer requirements. Note: Some tests may have been performed as part of the installation process. Phase 5 - Project Closeout Tasks: • Complete and deliver site -specific Lewan Post Installation Report • Review the configuration document with Customer • Obtain Customer signoff on Project Completion Form (Appendix A) Work Prerequisites The Work Prerequisites for services under this SOW are: • Lewan must receive one original copy of this document with an authorized signature and purchase of a time block and/or credit approval authorization. • The customer must designate a single point of contact and a backup contact for communications with Lewan personnel. • Upon receipt of the signed authorization, credit approval and designated contact, this engagement will commence as specified under the Project Timeline as agreed by both parties. Unless otherwise stated within this document, all dates and times are given to serve for the purpose of reference only and will not be used against either parry for award of payment or deliverables. Lewan & Associates 4 Scope • Lewan will provide Hardware installation services for the Lefthand Networks IP-SAN. A clustered configuration of Storage Modules will be created, with "ALB" Bonded Ethernet connections will be created and tested. • Configuration of VLANs are not covered in the statement of work. Installation and configuration of iSCSI drivers on up to five systems running supported OS types: Windows200012003, LINUX 2.4.2xx, and Netware 6.5SP1. • Presentation and formatting of up to three IP-SAN LUNs per server. Lewan will coordinate planning and design sessions with IT personnel to gather specific design and deployment requirements and communicate any preparation that is required by the customer. This will provide information to Lewan, and City of Fort Collins for site preparation and design details that will be agreed upon between Lewan and City of Fort Collins. This information will be utilized as part of the implementation plan. Deliverables • Installation and configuration of IP SAN hardware. • Installation and configuration of iSCSI drivers. • Presentation and formatting of IP-SAN volumes • Successful Data Migration of existing NSM160 data to the new DL320 environment. • Documentation will be provided for all customizations performed Customer Responsibilities • City of Fort Collins is responsible for the backup of all data and verification of those backups prior to commencement of this storage project. • Lewan & Associates requires access to City of Fort Collins personnel for requirements gathering, scheduling, project status and materials review. • All servers must have power available, and ready network connectivity, including appropriate name resolution services. • City of Fort Collins will perform all operating system activities and new installations/configurations with input from Lewan resources. Lewan & Associates will work with City of Fort Collins project team to conform to internal standards. • Provide roottadmin-level access to all servers involved for direct assistance from Lewan resources. • Provide necessary hardware for patch installation, typically DVD-ROM and Internet. • Provide physical access to the systems involved. • Provide support identifiers for contacting vendor support contracts, and/or provide someone to control and manage the dialog with the vendor support resources. • Install all OS, application and database software and patches according to project plan NOTE: Lewan will work closely with City of Fort Collins personnel on all software installations. • Provide necessary license information/documentation to accomplish installation. • Provide single point -of -contact for Lewan to coordinate with for this engagement. This person should have knowledge of the target systems, and the authority or timely access to the necessary decision -making authority. • Promptly coordinate and schedule system outages, which are needed for hardware installation, patch installation, and testing, etc. These events will be scheduled to avoid project delays, while minimizing user impact. • Provide adequate work area for Lewan personnel, including electrical power, telephone access, and either Internet access or analog telephone line for the electronic retrieval of patches, etc. Lewan & Associates Project Costs We will use the appropriate blend of systems engineers to deliver this engagement. All terms and conditions in this proposal are valid for 30 days from the date of this proposal, unless extended in writing by Lewan & Associates. Fixed cost for LPS consulting services during normal business hours: $4,200 • Any work performed after business hours or on weekends will be billed at Overtime and Holiday/Sunday rates as stated in the signed Lewan Pricing Agreement for the "Storage Engineer". Travel for Overtime and Holiday/Sunday is billed at %2 the round trip driving time. • At the customer's request Lewan may provide fixed cost pricing for after business hours services. • Any work performed outside of the scope of this Statement of Work will be billed time and materials at the engineering rate(s) outlined in Customer's Lewan Pricing Agreement. Project Timeline All dates unless agreed upon in advance are subject to change. A minimum of a two -week notice may be required to schedule resources. For questions regarding Lewan Professional Services (LPS) resources scheduling, please contact the Technical Services Client Support Center at 888-LEWAN88 (888-539-2688). Project Management / Personnel Sales Consultant — Steve Cluff Consulting Engineer —Dennis Laube Call Center — All calls and requests for paging personnel for service/support should be placed to the Technical Services Call Center at 888-LEWAN88 (888-539-2688). The call center hours are M-F, 7:30am - 5:OOpm (excluding company holidays). A message can be left outside the normal hours of operation and will be returned the next business day. Lewan & Associates Considerations When working in, maintaining or resolving issues in an existing IT environment, there is always the risk of data loss due to the failure of equipment or software. Lewan & Associates will do everything feasible to prevent any loss of data, as well as make effort to avoid system failure or downtime. Lewan & Associates cannot be held liable or accountable for the loss of data or cost of reconstruction should they occur during the course of this agreement. We recommend that customers create, verify, and maintain backup copies of all data prior to services being rendered. Lewan & Associates is rendering services in an advisory and service capacity. Lewan personnel strictly adhere to the licensing terms and conditions of all manufacturers' license agreements. Our responsibilities for installing software and hardware, and for product warranties, are limited to the manufacturer's specifications and terms. Technology advances have created an environment where many manufacturers as well as vendors do not have the opportunity to test every possible combination of hardware and software. Lewan & Associates has established key vendor relationships with hardware and software vendors to ensure the highest level of technical support in order to resolve complex issues. Lewan Systems Engineers will utilize all levels of technical support to ensure that services are performed effectively and efficiently. Our Systems Engineers will provide ongoing communications with the customer regarding unresolved or complex issues and additional time that may be required to resolve them. Lewan & Associates will provide no charge telephone support for services performed by us for (30) days after the Statement of Work has been completed. This support is limited to problem determination and resolution related to services directly performed by Lewan and does not include network skills transfer or application support, unless otherwise defined within the Statement of Work. Request for on -site support will be provided at block rate with a one - hour minimum, unless otherwise defined within the Statement of Work. Statement of Work Changes and / or Additions Lewan & Associates engineers are required to have a change order for any additional services to be performed that are not outlined in this Statement of Work. Lewan offers a wide variety of consulting and engineering services. We will partner with your Company to help you meet every business objective possible. Please make us aware of any changes and/or additions to this Statement of Work, or your business needs. All Statement of Work changes and/or additions will be made in writing prior to engineers performed said services. In an effort to provide you with the most complete solutions, Lewan Engineers will make Customer aware if any of the following occur: • Engineer(s) find Statement of Work and/or scope changes • Engineer(s) become aware of any "best practices" technical information which Customer should be informed of (i.e. upgrades, patches, recommended practices) Upon identification of any of the above scenarios, Customer and Lewan will agree on the course of action. Lewan & Associates will then proceed to generate a new Statement of Work or a Scope Change Document. The new services will not be performed until the parties have executed a Scope Change Document or new Statement of Work. Lewan & Associates
