HomeMy WebLinkAboutCOLORADO MOSQUITO CONTROL INC - CONTRACT - RFP - P1059 INTEGRATED PEST MANAGEMENT - MOSQUITO CONTROL SERVICESSERVICES AGREEMENT THIS AGREEMENT made and entered into the day and year set forth below by and between THE CITY OF FORT COLLINS, COLORADO, a Municipal Corporation, hereinafter referred to as the "City" and Colorado Mosquito Control, Inc. hereinafter referred to as "Service Provider". WITNESSETH: In consideration of the mutual covenants and obligations herein expressed, it is agreed by and between the parties hereto as follows: 1. Scope of Services. The Service Provider agrees to provide services in accordance with the scope of services attached hereto as Exhibit "A", consisting of Twenty (20) pages, and incorporated herein by this reference. 2. Contract Period. This Agreement shall commence April 1, 2007, and shall continue in full force and effect until March 3, 2008, unless sooner terminated as herein provided. In addition, at the option of the City, the Agreement may be extended for additional one year periods not to exceed four (4) additional one year periods. Pricing changes shall be negotiated by and agreed to by both parties. The Denver - Boulder CPI-U as published by the Colorado State Planning and Budget Office will be used as a guide. Written notice of renewal shall be provided to the Service Provider and mailed no later than ninety (90) days prior to contract end. 3. Delay. If either party is prevented in whole or in part from performing its obligations by unforeseeable causes beyond its reasonable control and without its fault or negligence, then the party so prevented shall be excused from whatever performance is prevented by such cause. To the extent that the performance is actually prevented, the Service Provider must provide written notice to the City of such condition within fifteen (15) days from the onset of such condition. rev 09/01/06 1 EXECUTIVE SUMMARY Colorado Mosquito Control, Inc. (CMC) is pleased to offer the following bid proposal for the 2007 City of Fort Collins Mosquito Management Program. CMC understands that the City of Fort Collins seeks to provide the highest quality public services to its residents. We believe that CMC has proven our ability to provide such high quality cost-effective services and professional working relationships over the past 4 years, and hopes to continue this service in 2007 and well into the future. West Nile Virus cases waned on the Front Range between the 2003 epidemic and 2005, but the summer of 2006 showed quite emphatically that WNV is here to stay. Here in the City of Fort Collins, the high numbers of Culex tarsalis mosquitoes and the resurging patterns of endemic WNV, cause mosquito control and surveillance to remain critically important for the protection of city residents. This proposal has been designed not only with limiting WNV activity (and other mosquito -borne diseases) in mind, but with a solid Integrated Pest Management (IPM) framework. The mosquito management program offered by CMC, addresses not only disease vectors, but all mosquito species that may vector future diseases and that could decrease the quality of life for residents on a daily basis. In the years since the City of Fort Collins Mosquito Management Program's inception and hiring of CMC as the program contractor, the Mosquito Management Program, under the operation and management of CMC, has developed into one of the foremost environmentally sensitive and technologically advanced Integrated Mosquito Management Programs in the United States. Additionally, CMC has fostered the cooperative efforts for mosquito control and epizootic/epidemic response management between the City, Larimer County Department of Health and Environment, the City of Loveland, other city municipalities, and The Centers for Disease Control and Prevention, Vector -Borne Disease unit here in Fort Collins. The value of this cooperative program and its underlying data sharing and communications cannot be over -emphasized. Colorado Mosquito Control has designed, implemented, pioneered, refined and pushed to new technological levels, a comprehensive (and cooperative) mosquito control program based on the sound scientific principles of IPM and the environmental goals of The City of Fort Collins. In addition to the operational objectives of environmentally sound larval and adult mosquito control, CMC has also successfully implemented public education and informational programs to help residents discover what benefits modern mosquito control can have on public health and quality of life in their community. Other important support services have included: a diverse informational Website, 2006 Call Notification and Shutoff program, GIS layers providing resident address and location for notification and shutoff requests, and implementation of CMMS1m, CMC's proprietary and unique historical database system, as well as a GIS (Geographic Information System) that works in conjunction with the City of Fort Collins, City of Loveland, and Larimer County GIS systems. These services have expanded the scope and breadth of the City of Fort Collins Mosquito Control Program operation dramatically over the last 4 years. CMC has a 20 year history of offering high quality, large scale, full service mosquito control and disease surveillance programs to numerous Colorado municipalities. CMC has survived and enjoyed significant growth over time with no marketing or sales promotions. There are several good reasons for this fact that include: the honesty and integrity of our employees, experience and expertise in our field, investment in technology and training, and the cost-effective high quality services that we always provide. 2007 City of Fort Collins Mosquito Control Services Proposal RFP-P 1059 Colorado Mosquito Control, Inc. Page 3 The following proposal will extend the same high quality surveillance, control and supplementary services that CMC has successfully built and operated for the City of Fort Collins over the past 4 years and in surrounding Front Range communities for over 20 years. Thank you for the opportunity to submit this bid proposal. SECTION I A. PROJECT DESCRIPTION Colorado Mosquito Control, Inc. will conduct a comprehensive Integrated Mosquito Management (IMM) Program for the City of Fort Collins (as defined below), beginning in March 2007. Colorado Mosquito Control, Inc. will employ established IMM principles and practices to dramatically reduce the numbers of mosquitoes in the City of Fort Collins. These principles and practices will consist of an environmentally sensitive program that utilizes a combination of cultural, biological and feast -toxic chemical measures that emphasize the environmental approach, to target and control all mosquitoes, including potential disease -vector populations and nuisance species, and which is economically feasible, all with an emphasis on eliminating or minimizing chemical pesticide applications. B. PROJECT AREA Colorado Mosquito Control understands and agrees that the City of Fort Collins Mosquito Management Program consists of the geographic area including the City of Fort Collins and an appropriate buffer area surrounding the City. CIVIC expects the program area to be approximately 109 square miles and understands that the size and scope of the program is subject to change. Page 4 No Text SECTION II INTEGRATED MOSQUITO MANAGEMENT PROGRAM SCOPE OF WORK 1. Legal and Financial Requirements a. Advanced Pest Management of Colorado, Inc. d/b/a Colorado Mosquito Control, Inc. is currently licensed and will maintain licensing to perform commercial mosquito control services in the State of Colorado. Colorado Department of Agriculture License No. 10261. See APPENDIX 1 for a copy of the License and Endorsements. CMC currently employs 13 Colorado Department of Agriculture Licensed Qualified Supervisors. Colorado Mosquito Control will meet all Federal, State and local requirements for the storage, transport, dispensing and disposal of all products, including pesticides used in the City of Fort Collins Mosquito Management Program. b. Colorado Mosquito Control, Inc. has the financial status, condition and capital equipment to provide the services described in this proposal. 2. MosquitoLine"" CMC will continue to maintain a toll -free (in Colorado) telephone line: (877) 276-4306, and a local Fort Collins phone number; 970-663-5697 (at no cost to the city) to accept calls from the public concerning, but not limited to the following: a. Report mosquito annoyance areas, and/or standing, stagnant water that may indicate the presence of larval mosquito sites or harborage. b. Opt their property out of any adulticide spraying via a shut-off list c. Request notification when adulticide spraying is planned in and around their neighborhood. d. Request fish to control mosquito larvae (where applicable and appropriate as determine by CMC management). e. Request information on how to control and/or prevent mosquitoes on their property and mosquito -borne diseases such as WNV, WEE and SLE. f. Request health and safety information about mosquito control operations and pesticide products used in the City of Fort Collins Mosquito Control Program. g. Request information regarding larval treatments performed on reported and inspected larval sites. CMC has provided Mosquito Hotlines for the residents of Fort Collins, to also reduce workload by municipal personnel. This enables direct communication and response by mosquito control employees to resident concern about West Nile Virus and larval site activity and treatment. CMC will maintain a log of calls received and will summarize call activity in monthly and annual reports. 3. I m med i ate Response"" a. CMC introduced the concept of a 24 hour guaranteed response/resolution time to all mosquito annoyance complaints in 1994 with our trademarked ImmediateResponseTM System. CMC will continue to respond appropriately to all mosquito related complaints and will refer any service related complaints to the City of Fort Collins Project Manager. 2007 City of Fort Collins Mosquito Control Services Proposal RFP-P1059 Colorado Mosquito Control, Inc. CMC will verify the validity of mosquito annoyance complaints by on -site inspection including; adult mosquito trapping via nearby pre -determined trap locations or floater traps, as needed, and/or landing counts (10 females mosquitoes per 5 minute period = annoyance level) and/or larval dip surveillance in local breeding sites. c. CMC will resolve all complaints, if possible within 24 hours. d. CMC will provide education, either verbally or through educational materials to the complainant in an effort to promote self management of mosquito problems. e. CMC will continue to maintain a database of all calls, complaints and report resolutions. 4. Adult Mosquito Trap Surveillance a. CMC will conduct intensive adult mosquito trapping during the mosquito season (approximately late May through mid September). Trapping of adult mosquitoes at minimum will include: i. Weekly trapping at 41 pre -determined CDC light trap surveillance locations within the City of Fort Collins and 5 pre -determined gravid trap locations within the City of Fort Collins. There are two additional light trap surveillance locations within the City of Fort Collins, with trap counts reported weekly to the City of Fort Collins, but are funded separately by Homeowners associations within the City. ii. An appropriate number of Floater Traps for use in areas in the Program that are not covered by one of the 48 predetermined trap locations. Floater traps will be used to determine adult mosquito numbers in areas where complaints regarding high numbers of mosquitoes have been received and to determine whether spraying for adult mosquito control is justified (see Adult Mosquito Control, below.) iii. Both CDC light traps and gravid traps (where applicable) will be used. iv. Speciation and counts of adult mosquitoes will be completed within 24 hours of trap operation. Counts using statistically valid sampling methods will be used in large trap sample sizes (over 300 mosquitoes/trap). v. CMC will ensure that speciated Culex (vector) mosquitoes will be made available for submission to the CDPHE/ CDC Laboratory for disease testing (when applicable) within 24 hours of trap retrieval. vi. CMC will maintain trap data via our trademarked CMMSTM System and will provide reports as described below on a weekly basis. vii. All mosquito trapping activities including the submission of mosquito pool specimens will be conducted in accordance with the protocols established in the CDPHE 2007 Mosquito Surveillance Plan. viii. Colorado Mosquito Control, Inc. will assume all liability for the placement and operation of any and all equipment. The City of Fort Collins will not be held responsible for any lost, damaged or stolen traps and/or equipment. 2007 City of Fort Collins Mosquito Control Services ProposaL RFP-P 1059 Colorado Mosquito Control, Inc. Page 7 CMC'S MOSQUITO SURVEILLANCE & LABORATORY RESOURCES Ar In -House Larval and Adult Mosquito Identification is critical to species ID accuracy. CMC has performed larval Field ID in past years, but upon study and direct comparison, we have found that Field ID is much less accurate and much more time consuming than Lab ID that is performed by specialists trained in Lab ID and who have access to high quality stereo zoom microscopes. Management: All CMC surveillance and laboratory services are managed by Dr. Michael "Doc" Weissmann. Dr. Weissmann holds a PhD in Entomology from Colorado State University. Dr. Weissmann is a renowned world-wide expert in Dipteran and Lepidopteran biology and taxonomy. V Personnel: CMC (Larimer Office) has 3 full-time field surveillance and laboratory technicians trained in mosquito trap operations and taxonomic identification of larval and adult mosquitoes to species level. !r Equipment: The CMC Surveillance Laboratory (Brighton) is fully equipped with 4 stereo zoom binocular microscopes, (2 additional in Larimer Office) including one video/photographic microscope capable of projecting microscopic images onto computer screens or to projection equipment for wall sized viewing. This is the perfect tool for identification training and presentations. CMC owns and maintains over 60 CDC dry -ice baited Light Traps ind 21 Reiter Gravid Traps and all associated equipment and irdware. See Appendix 5, Capital Equipment List for more ,rmation, Infrastructure: CMC has over 500 sq ft. of devoted Surveillance Laboratory facilities and an additional 300 sq. ft. of trap maintenance and dry ice storage space. In addition to the above mentioned equipment, CMC's lab faculties (Larimer Office) include other important equipment such as a deep freezer, 2 computer workstations, mobile dry ice holding container, and a fleet of 2 trucks specifically designated for surveillance operations. 2007 City of Fort Collins Mosquito Control Services Proposal RFP-P1059 Colorado Mosquito Control, Inc. Page 8 ti Technology: All data generated and collected by the Surveillance and Laboratory staff is entered into the CMMSTI database system within 24 hours of collection. This allows rapid reporting of results to our clients and field operations managers which allows them to respond quickly to emerging field situations and "hot -spots". 2007 City of Fort Collins Mosquito Control Services Proposal RFP-P1059 Colorado Mosquito Control, Inc. Page 9 S. CMMSTm LARVAL MOSQUITO MAPPINGI SURVEILLANCE & CONTROL A. CMC has traditionally focused over 95% of control efforts in the City of Fort Collins on larval mosquito surveillance and control with great success and will certainly continue. Over the past 4 years CMC has identified and mapped over 1,198 known and potential mosquito -breeding sites within the City of Fort Collins and in the surrounding Larimer County buffer zone. In the last two years alone CMC has found and added over 261 new sites. CMC will perform larval mosquito control operations from late March through September (as needed, dependant on weather and presence of larvae). CMC, utilizing the CMMSTM system, will continue to maintain a larval site history database that includes a site catalog number, date and time inspected, if site was wet or dry, treatment (if any), product type, amount, estimated acreage treated, larval density, larval instars present and larval Genus and species (added by CMC Surveillance Laboratory staff after ID of field collected larval sample). CMC has continually updated the CMMSTm database system and associated GIS data layers with additional new-found breeding sites for the past 4 years and will continue to add sites as they are identified. B. CMC will utilize the CMMSTM system to accurately and efficiently target larval mosquito control efforts, and will include: I. CMMSTm targeted larval site inspections, as needed, to all previously located and new found potential larval habitats. Weekly site inspection will be based on proprietary CMMSTm algorithms that select and target larval sites based on past comprehensive site history data. This data incorporates historic records of larval presence, larval densities, larval genus and species, habitat size and type, water source, and weather data. II. As sites are found to be breeding mosquito larvae, CMC will first look to cultural or physical controls to reduce standing water and larval development. Water management and source reduction including clearing clogged ditches, hand digging of channels, and working with property owners to mitigate these sites will be included. III. CMC will then, as needed, treat sites found to be breeding mosquito larvae with a "Prescription -oriented" least -toxic, biological treatment appropriate to the site, larval density and history of the site. Larval control products will include; Vectobac (Bti), Vectolex (Bs), Altosid (methoprene) in catch basins and storm drains, and Bonide (highly refined mineral oil). Application will be at label rates, at industry standard rate of 5.0 lbs. per acre, for dry products. IV. Urban Backyard Program. CMC's specially trained backyard technicians will work closely with citizens in residential areas to identify and eliminate localized larval mosquito development. V. CMC will inspect and treat as needed, approximately 5230 storm water catch basins throughout the mosquito season within the City of Fort Collins utilizing our unique urban storm - water program. VI. CMC will distribute Fathead minnows Pimephales promelas (or other approved species) to City residents, individually for use in ornamental ponds and stock tanks. 2007 City of Fort Collins Mosquito Control Services Proposal RFP-P 1059 Colorado Mosquito Control, Inc. Page 10 Fish Give -Away Public Event. CIVIC will work with the City to organize, promote and hold a "Fish Give -Away" Event. CIVIC has successfully operated recent Fish Give - Away Events in Fort Collins, Loveland and Broomfield, and has given away well over 20,000 Fathead minnows (Pimephales promelas) to event goers for mosquito control in their ornamental and closed -system ponds. Unlike Gambusia affinis (mosquitofish), Fathead minnows are indigenous to Colorado and unlike Fundulus zebrinus (plains killifish) are not on the threaten -species list. Fathead minnows (Pimephales promelas) are recommended and preferred for use in this region of Colorado by the Colorado Division of Wildlife. C. Hot Spot Targeting. CIVIC has traditionally recognized specific areas within the City of Fort Collins in which a high number of mosquitoes are trapped consistently from one trap week to the next. CIVIC will continue to allocate additional resources for finding the sources of these mosquitoes and utilize the appropriate biological treatment of larval mosquitoes with the goal of lowering the number of mosquitoes to preclude needs for adulticide applications. When access to a property or "hot spot" cannot be gained, CIVIC will notify the City of Fort Collins Project Manager of these areas. 2007 City of Fort Collins Mosquito Control Services Proposal RFP-P 1059 Colorado Mosquito Control, Inc. Page I "r Personnel: CIVIC will devote full-time field technicians to performing IMM-based larval mosquito control in the City of Fort Collins Mosquito Control Program. These techs are fully trained via CMC's classroom and field training program and well exceed the minimum training standards set by the Colorado Department of Agriculture. Management of the program is by Jessica Schurich, a four year CMC veteran employee who holds a BS in Environmental Studies and concentration in Animal Behavior from East Stroudsburg University. �r Equipment: Every technician has his or her own assigned CMC-owned fleet vehicle which is fully equipped with all necessary larval surveillance equipment, larval control application spray equipment, and biological and bio- chemical larvicide products. v Infrastructure: Since over 95% of CMC's operations are targeted toward Larval Mosquito Control, that same percentage is applied in infrastructure to facilitate those operations. CMC's warehouse, material handling equipment, supply chain, data input, vehicle fleet, and application equipment are all designed to support our larval control operations. Z Technology: CMMST`" CMC's proprietary and unique historical database system will be used to analyze historical breeding site data, to target site inspections and microbial larvicide control applications. 2007 City of Fort Collins Mosquito Control Services Proposal RFP-P1059 Colorado Mosquito Control, Inc. Page 12 4. Early Termination by City/Notice. Notwithstanding the time periods contained herein, the City may terminate this Agreement at any time without cause by providing written notice of termination to the Service Provider. Such notice shall be delivered at least fifteen (15) days prior to the termination date contained in said notice unless otherwise agreed in writing by the parties. Attn: Michael McGinnis 695 North 71h Ave. Brighton, CO 80601 In the event of early termination by the City, the Service Provider shall be paid for services rendered to the date of termination, subject only to the satisfactory performance of the Service Provider's obligations under this Agreement. Such payment shall be the Service Provider's sole right and remedy for such termination. 5. Contract Sum. The City shall pay the Service provider for the performance of this Contract, subject to additions and deletions provided herein, according to Exhibit B, attached hereto, consisting of one (1) page and incorporated herein by this reference, the sum of Two Hundred Thirty Thousand Six Hundred Sixteen Dollars ($230,616). 6. City Representative. The City will designate, prior to commencement of the work, its representative who shall make, within the scope of his or her authority, all necessary and proper decisions with reference to the services provided under this agreement. All requests concerning this agreement shall be directed to the City Representative. 7. Independent Service provider. The services to be performed by Service Provider are those of an independent service provider and not of an employee of the City of Fort Collins. Page 13 6. WORK PRODUCT OWNERSHIP, RECORD KEEPING AND REPORTS a. All data generated by and collected during the performance of the City of Fort Collins mosquito control program becomes the sole property of The City of Fort Collins. b. CMC will provide program performance data to The City of Fort Collins in a common storage media such as a CD/DVD in a common, manipulative, electronic/digital format. c. CMC will submit an Annual Report to the City by December 15th of the current contract year. d. The annual report will contain: i. An overview of the control efforts for that year ii. Number of larval sites inspected, number sites treated, and acreage treated iii. Amounts of each larval control agent used iv. Composite of mosquito species trapped both for light traps and gravid traps City wide and for each trap v. Annual ULV miles and the location of each ULV site indicating, date, material used, start time, end time and miles vi. Total annual calls received and type of call by month vii. Mapping details: 1. Trap Sites 2. Levels of larvicide treatment 3. Area spraying in a mapping format (to be in a format capable of being reproduced by the City's GIS Department) e. CMC will submit Monthly Reports of trapping activities, control efforts during the months of March -October by the 15th of the following month. f. Weekly Summary Reports & Monthly Reports (See Appendix 6 for Sample Report) will include: i. Number of Sites Inspected ii. Percent Sites Wet iii. Percent Sites Positive (number of sites with larvae divided by number of sites wet) iv. Number of Sites Treated v. Number of Acres treated with larvicide products vi. Weekly adult mosquito trapping data vii. Total number of mosquitoes per surveillance site viii. Numbers of mosquitoes per Genus species (Aedes, spp., Culex spp., Culiseta spp., & Coquitlettidia spp.) per site ix. Page 14 U Personnel: CMC has dedicated office staff to handle all incoming telephone communications, data input via state-of-the-art TeleForm© scannable form reading software, and communications with field technicians via our Nextel Direct -connect walkie- talkie system. i+ Equipment and Technology: We have a state-of-the-art Dell Server supported computer network, numerous Dell workstations, printers, faxes, and scanners and T-1 high-speed internet access, all designed to support our CMMSTm and ArcGis software system. At CMC, we view Technology as the key to continued effective, efficient, cost - beneficial and environmentally sound mosquito control for today and well into the future, lo **NEW** Digital Interactive Reporting: CMC continues to pioneer new ways to both accomplish and report on our mosquito control services. Recently CMC introduced a radical departure from traditional reporting methods: Digital Interactive Resorting. CMC will attend The City of Fort Collins City Council or Project Management Team meetings and any other public outreach efforts when requested by Project Manager. b. CMC will continue to maintain our website (www.comosguitocontrol.com) c. CMC will continue to provide Education and Public Outreach for City residents in an effort to help reduce or control nuisance and disease vector mosquitoes around their home through the use of our comprehensive web site (www.comosquitocontrol.com) or during personal interaction. This website will continue to support links to the City and LCDHE websites. d. Colorado Mosquito Control, Inc. will to work on and assist city staff in developing printed or electronic public education informational materials relating to the City of Fort Collins Program or public health related mosquito -borne disease topics such as WNV. e. CMC maintains and will continue to maintain website links to LCDHE (Larimer County Department of Health and Environment), CDPHE (Colorado Dept. of Public Health & Environment) and CDC (US Centers for Disease Control). Contingency Adulticide Procedures and Outreach f. CMC will communicate recommendations from LCDHE for adulticiding applications and gain permission from the City of Fort Collins City Manager and the Project Manager prior to any adulticide application. g. CMC will notify residents on the Call Notification/Shutoff List when conducting any City requested ULV adulticide spray applications within 2 blocks of their property or within the effective ULV spray drift distance (300-500 ft depending on wind speed and direction). Call & Shutoff forms are available online for download and may be submitted via the CMC website. h. Call Notification will be given within 24 hours of routine adulticide applications or 48 hours in advance of any "city-wide" applications i. Contractor has participated or attempted to participate in the basic pilot employment verification program created in Public Law 208, 104th Congress, as amended, and expanded in Public Law 156, 108th Congress, as amended, administered by the United States Department of Homeland Security (the "Basic Pilot Program") in order to verify that Contractor does not employ any illegal aliens. B. Contractor shall not knowingly employ or contract with an illegal alien to perform works under this Agreement or enter into a contract with a subcontractor that fails to certify to Contractor that the subcontractor shall not knowingly employ or contract with an illegal alien to perform work under this Agreement. C. Contractor shall continue to apply to participate in the Basic Pilot Program and shall in writing verify same every three (3) calendar months thereafter, until Contractor is accepted or the public contract for services has been completed, whichever is earlier. The requirements of this section shall not be required or effective if the Basic Pilot Program is discontinued. rev 09/01/06 5 D. Contractor is prohibited from using Basic Pilot Program procedures to undertake pre -employment screening of job applicants while this Agreement is being performed. E. If Contractor obtains actual knowledge that a subcontractor performing work under this Agreement knowingly employs or contracts with an illegal alien, Contractor shall: 1. Notify such subcontractor and the City within three days that Contractor has actual knowledge that the subcontractor is employing or contracting with an illegal alien; and 2. Terminate the subcontract with the subcontractor if within three days of receiving the notice required pursuant to this section the subcontractor does not cease employing or contracting with the illegal alien; except that Contractor shall not terminate the contract with the subcontractor if during such three days the subcontractor provides information to establish that the subcontractor has not knowingly employed or contracted with an illegal alien. F. Contractor shall comply with any reasonable request by the Colorado Department of Labor and Employment (the "Department") made in the course of an investigation that the Department undertakes or is undertaking pursuant to the authority established in Subsection 8-17.5-102 (5), C.R.S. G. If Contractor violates any provision of this Agreement pertaining to the duties imposed by Subsection 8-17.5-102, C.R.S. the City may terminate this Agreement. If this Agreement is so terminated, Contractor shall be liable for actual and consequential damages to the City arising out of Contractor's violation of Subsection 8-17.5-102, C.R.S. H. Scope of Services Survey & Mapping Services 1.1. Survey land with in the larval control area (approximately 109 square miles) to identify and map mosquito breeding sites. 1.2. Develop and maintain mosquito control service related information in GIS format. 1.3. Provide GISD maps indicating mosquito breeding sites and larval control activity. 2. Mosquito Breeding Site Inspection & Larviciding Services 2.1. Inspect Mosquito breeding sites with appropriate frequency to determine the need to apply larvicide to control mosquitoes. 2.2. Apply larvicide as needed through the mosquito control season. The City expects that approximately 2000 acres with in the larval control area will require application of larvicide on an on -going basis through out the mosquito control season. Larvicide treatments may be applied through a variety of appropriate methods. 2.3. Provide periodic follow up Quality Control follow-up inspections of larvicide applications. 3. Open Street Storm Drain Catchbasin Inspection & Larviciding Services 3.1. Inspect and treat approximately 5230 street drain catchbasins with larvicide through out the mosquito control season. 4. Adult Mosquito Surveillance Services. 4.1. Design and conduct an on -going mosquito surveillance program utilizing CDC Light Trapping and Reiter Gravid Trapping methods. 4.2. Provide Laboratory Species Identification services. 4.3. Provide mosquito samples to State and/or CDC for WNV testing as necessary. 5. Public Relations & Education Services 5.1. Accept calls from the general public regarding possible mosquito breeding site locations and mosquito control services and issues in general on an on -going basis. 5.2. Provide information to the news media regarding the mosquito control issues as appropriate. 6. Reports 6.1. Provide the City with on -going status reports including: 6.1.1. Weekly surveillance trap reports including trap counts, 6.1.2. Larviciding activity reports, and , 6.1.3. Telephone Call log reports. 6.1.4. Monthly summary reports 6.2. Provide the City with an Annual Report summarizing the mosquito control program activity for the year at the conclusion of the mosquito control season.
