We are prepared to provide professional installation services and support to The City of Fort Collins. If possible, we recommend that the City of Fort Collins perform onsite interviews with the AV finalists vying for this project. There were some critical pieces of equipment left out of the original bid specification. We have included these missing pieces in our proposal and we welcome a thorough discussion of our system design, equipment details and user interface issues. Our proposal and cost summary reflects pricing as detailed in the RFP. It also details equipment required to make the system function as described in the narrative and consequent addendums. CEAVCOM/AYMO Engineering is also open to providing some value engineering suggestions that will provide the same (if not better) end result for less money. Our experience with other AV projects of this magnitude has given us hands on knowledge of what works ok and what works even better! We acknowledge receipt of addendums 1 — 3. Final equipment list, project scope of work, and/or project drawings will be submitted for your approval and signature. Electrical work, conduit, junction boxes, floor boxes, raceways, and core drills are not part of this proposal and will need to be provided and installed by others. Invoices detailing product acquired and/or services provided will be generated on a monthly basis. Equipment being warehoused by CEAVCO will be itemized with serial numbers, and available for inspection. Invoices or applications for payment are due 30 days from the date of invoice or application date. ADDITIONAL PROVISIONS This quote is valid for 30 days; thereafter it is subject to change without notice. There will be a 25% restocking charge for equipment returned because of customer requested changes to scope of work. Video Conferencing systems require a network and associated devices such as switches, firewalls, routers etc. which we do not furnish and for which we are not responsible. Upon completion of the installation of the equipment we furnish, we will demonstrate the integrity of our installation. (Manufacturer warranties do not include this on -site labor). Extended One -Year CEAVCO On -Site Limited Warranty • Extends 90-day on -site service warranty stated above to one full year • Provides one Scheduled Preventive Maintenance visit during the first year. Please read the attached Limited Warranty Contract and sign to indicate your acceptance. This policy will take effect upon our receipt of payment or a purchase document to authorize invoicing. N *CEAVCO AudioVisual CEAVCO Audio Visual Company, Inc. 6240 W 54th Ave Arvada, CO 80002 303 539 3400 Fax 303 539 3401 LIMITED WARRANTY POLICY www.ceavco.com We hereby warrant that the audio-visual system we have supplied and installed for: Customer Description of Installation System Installation Warranty -One Year The above -described system(s) has been completed in accordance with the approved Scope of Work, drawings and specifications. CEAVCO Audio -Visual Company, Inc. agrees to repair or replace, at our option, any or all of our work associated with the installation of the equipment we supplied, that may prove to be defective in its quality -of -work within a period of one year from Substantial Completion (the date at which system is used for its intended purpose, regardless of functionality). The individual components of the system carry the manufacturer's warranty and are not covered under this warranty policy. CEAVCO shall not be liable for any damages, including incidental or consequential damages, expenses, costs, profits, lost savings or earnings, loss of computed data, or other damages resulting from CEAVCO's workmanship, or the misuse of the equipment or items installed added, changed, moved, or repaired by us. This warranty is limited to the correction of the problem and does not apply when a problem is due to force, normal wear and tear, damage, misuse, negligence, improper line voltage, product setups or adjustments by purchaser, conveyance and purity, routine maintenance, customer education or abuse by the original purchasers or their employees, agents or representatives. The warranty does not apply to existing items that were installed, added to, changed, moved, repaired, or altered from the original design or performance standards by anyone other than a CEAVCO employee. This warranty service is available during regular business hours Monday through Friday 8:00 a.m. to 5:00 p.m. excluding CEAVCO holidays. During the first 90 days of system operation CEAVCO will provide up to three on -site emergency service visits. We will remove and reinstall equipment requiring off site service at no additional charge. CEAVCO is factory authorized to repair most of the products we sell. Some service issues will require shipping equipment to factory service depots for repair, in such cases, CEAVCO will charge the customer for shipping and handling. Extended One -Year Limited Warranty, Including One Preventive Maintenance Visit Price S This provides one Scheduled Preventive Maintenance visit during the one-year warranty in addition to extending the above Standard 90-Day warranty for the first full year. The central room has a large rear projection system consisting of a Sony VPL-FX51 projector which displays on a 10' x 13' screen. Three Sony DXC390 cameras provide views of the audience and the board members sitting at the Dias. Multiple sources can be sent to the central screen, all five screens simultaneously, or any combination. Sources include videoconferencing, DVD, VHS, cameras, and multiple laptop feeds from the six Board members and secretary Dias seats as well as several podium locations. A 10" Crestron touch panel provides an easy to use interface for all source control, routing, volume control, lighting and videoconference functions. The four smaller side rooms each have Sony VPL-PX15 projectors and ceiling mounted screens. They share two mobile source racks with 6" touch panel control systems. These racks can be plugged into numerous wall plates and floor boxes for independent operation. Creating Solutions from the Possibilities Since 1961 PROJECT PROFILE 2) ConAgra Foods Continental Divide Training Room irA*CEAVCO Audio Visual Scope of Work: The Continental Divide Room is the focal point of ConAgra Foods newly constructed facility in Greeley, Colorado. The main entrance of the building steers you towards glass windows overlooking this architecturally stylish room. ConAgra employees utilize their Continental Divide Room for training classes as well as company meetings. When used as a classroom/training room, the Continental Divide Room can seat up to 50 people. In order to accommodate company meetings of up to 500 people, presentations made in the room are routed to five monitors in the cafeteria as well as to the projection systems in two executive boardrooms. One goal for this room was to conceal the AN equipment as much as possible. What couldn't be concealed was to be accentuated with the help of the architects. Custom millwork surrounds the rear projection screen as well as the front and rear surround sound speakers. A video camera hides away on a ceiling lift when not in use. Student microphones, one for every two students, can be connected in the desktops when needed for large classes and video and audio conferences, or can be stored when not in use. A custom multi -media lectern on the stage contains both record and playback VCR's, a DVD player and a document camera, as well as the room main computer and monitor. Presenters can run the show from the 10" AMX touch panel at the lectern or from a wireless 5" touch panel anywhere in the room. The control system runs everything from video and audio conferencing to the lights and drapes. Other technology features incorporated in this room include a Tech Electronics bulletin server tied to monitors in the lobby, cafeteria and fitness center, a satellite, feed, a GPS based digital clock system that runs throughout the building, a video connection from the front of the building to record and/or display VIP entrances, and two more laptop connections at the back of the room for "student" connections to the projection system. Each "student" location also has both network and AC power connections. Creating Solutions from the Possibilities Since 1961 PROJECT PROFILE 3) Jefferson County Emergency Operations Center ir�CEAVCO Audio Visual Scope of Work: CEAVCO Audio Visual Co. designed and installed an Audiovisual Presentation System for the Jefferson County Emergency Operations Center. The system, which is the focal point for displaying information for various emergency related functions, features a Crestron Control System. All functions of the system are controlled through a CT-1550 Color Touch Panel. The items controlled include a Draper Access V tensioned electric screen and an Epson PowerLite 8200i LCD projector including power on and off and switching of inputs. The Crestron system also controls output of multiple sources from an Extron SW4 RGB HV switcher with stereo audio to the projector and to the audio system. Those sources include a dedicated computer system through an Extron RGB 109xi interface, laptop computers brought into the facility through an Extron RGB 150x interface mounted under the surface of a desk, and a Samsung SPD-900DX (XGA) document camera through a wall mounted Inline CIA114 interface. The Crestron System controls Sony VCR and DVD players as well as switching projector inputs when they are selected. The Crestron Control system also controls audio levels from the VCR, DVD player and computer sources. Audio is supplied by a Crown XLS-202A stereo amplifier through a pair of JBL Control 25 wall mounted speakers. Shortly after completion of the installation, the facility was pressed into service when a heavy spring snowstorm dumped from 30 inches to over 6 feet of snow on Jefferson County. Prior to joining CEAVCO she worked as an on -air reporter and anchor for a WBBH-TV, an NBC affiliate in Fort Myers, Florida. She has also held positions as a producer and reporter for both Florida Public Television and National Public Radio. Tracey is a graduate of the University of Florida with a BS in Telecommunications. As a CEAVCO Sales Representative, Tracey is responsible for managing customer relationships, generating new business opportunities, and providing a detailed needs analysis and proposals for customers. Tracey knows the unique needs of video professionals both in the field and in the studio. She possesses strong video production and multimedia skills. Daryl Surface, CTS - Project Manager Daryl has been in the audio-visual, video and communications field for nineteen years. Before joining CEAVCO he was the audio-visual manager for the Marriott Corp in Denver and as a production manager at Hays Kansas' public access television station. Daryl has been with CEAVCO since 1986 and began as a sales representative. As CEAVCO'S Project Manager, Daryl is generally the primary customer contact during the installation phase of a project, and provides all installation scheduling. He facilitates the production of ESI supplied custom products such as pre -fabricated cables and panels. He also acts as a liaison with any additional contractors that effect a CEAVCO installation. This includes CEAVCO supplied contractors as well as client contracted electricians, construction crews, maintenance, etc. Daryl has been the head Project Manager on multiple projects including the Auraria Media Center, US West's executive conference room, the City of Lakewood, the City of Englewood, and TCI's corporate and Federal District Court. Daryl graduated from Ft. Hays State University with a BA in Communications, specializing in Television Production. Daryl has ICIA CTS Certification Gary Sutton, CTS -Senior Design Engineer Prior to moving to Denver Gary was Senior Design Engineer for a leading Dallas, Texas audiovisual integrator for seven years. While there he was responsible for the design of audiovisual systems for Motorola, Union Pacific Resources, University of Texas and Texas Health Resources among many others. During his tenure with ASC Companies in Dallas Gary provided technical support for company projects and salespeople. This included responsibility for designing and managing projects from initial concept design to final acceptance and training. 2 M Q CEAVCO AudioVisual From 1985 to 1996 Gary worked for cable television multiple system operators. While employed by TeleCable Corporation he provided company wide technical support for antomated commercial insertion systems, which included system design, installation and training. While employed by TCI Media Services he was responsible for MPEG-2 digital systems and computer based traffic and billing software. Gary graduated from Texas Tech University in Lubbock, Texas with a Bachelor's Degree in Telecommunications. His continuing education includes courses from Sony, Tektronix, Extron and AMX. Dennis Visser, CTS — Design Engineer Dennis attended Western Iowa Technical School graduating with an AS in Telecommunications. He also served in the United States Air force attending several Aviation Equipment Technical Schools. Dennis joined CEAVCO in October of 2000. Prior to joining CEAVCO he served as the Chief Engineer at KCEC for over 11 years. Before he left KCEC, Dennis had designed, engineered and supervised the installation of their newsroom, production facility, and master control room. He also maintained KCEC's broadcast video equipment and transmitter. Preceding his position at KCEC, Dennis was the Assistant Engineer at the Fox affiliate, KXRN channel 21, in Colorado Springs. As a System Engineer, Dennis leads large-scale projects from start to finish. His duties include preparation for arrival on the jobsite, managing installation crews, performing test and acceptance, and verifying that all punch list items have been completed. Dennis has had AMX, Extron, Clearone, and Crestron training. Eric Roster, CTS — Design Engineer / Certified Crestron Programmer Eric joined CEAVCO in January of 2001. His previous experience consists of Project Management at Jato Communications, and as a Technical Director for Up With People. Eric was employed at Up With People for 11 years and began as a Sound Technician working his way through the ranks to become a Technical Director. As a Technical Director, Eric supervised a staff of 15. Eric was responsible for the set-up and testing of their stages in North America and Europe. This set-up included sound, AV equipment, and lighting. As a System Engineer, Eric leads large-scale projects from start to finish. Eric has ICIA CTS Certification and is a certified Crestron Programmer April Steusloff — Installation Coordinator April is responsible for the purchase of Systems Group materials including cable, connectors, blank panels, rack shelves, cable ties, etc. She schedules installation people and coordinates site work with clients. April worked in the data and telephone installation industry with the Federal Government from 1985 until 1995. She joined CEAVCO in 1996. As the Installation Coordinator, she determines a project's cable and connector needs then stages these supplies for an installation crew to deliver to a job site. April also coordinates the staging of installation equipment with the Operations Department. Additionally, she functions as an installer and is skilled in punching patch bays, cable termination and dressing. She has had AMX, Crestron and AutoCad training. Guido Ceyssens — Lead Installer Guido is responsible for leading on -site crews and the day-to-day progress of installations of all sizes and types. He has 9 years experience in the audio-visual field. He was most recently the European Technical Manager for Up With People. Guido joined CEAVCO in December 2001. He brings both a strong management and mechanical background to CEAVCO. In addition to ICIA CTS Certification, Guido has successfully completed the ICIA Installer School. Russ Mann - Maintenance Engineer Russ graduated third in his class from the Milwaukee Area Technical College in 1975 in the Broadcast Engineering Program. He worked in the Milwaukee area, at television stations as a Broadcast Maintenance Engineer. From 1982 to 1995, he performed broadcast maintenance at Denver's CBS affiliate (Channel 7).
