current contract bid #5853 with Connell Resources to apply for this work.
Acceptance
Service Provider agrees to perform the services
identified above and on the attached foams in
accordance with the terms and conditions contained
herein and in the Services Agreement between the
parties. In the event of a conflict between or
ambiguity in the terns of the Services Agreement
and this work order (including the attached forms)
the Services Agreement shall control.

Connell will need to verify the pump layout prior to Installation.

Intake pipe indudes a gate valve upstream of the pump building for maintenance of the wet well.
24" condults are Included through the floor of the building and sweeps under the slab for electric and controls.
Medaical work Is excluded,
. construction water is to be pumped from the pond. Dewatering water Is to be discharged to the pond so that no dewatedng water is
discharged from the site.
. Please provide a minimum of 2 weeks advanced notification of work requests to allow for scheduling the work. No work will be completed
without a signed Purchase order Agreement. Wok Is anticipated to begin in December 2006.
. Frost removal or winter protection is not included. If requested, it will be billed either an a time and materiel Was or a lump sum amount
that is mutually agreed apon.
. Connell has not included prices for replacing damaged or broken concrete but will replace broken areas at unit prices per the Fort Collins
