PARTICIPATING PROVIDER AGREEMENT
THIS AMENDMENT, effective January 1, 2007, is entered by and between City of Fort Collins (Payor)
and Poudre Valley Health Care, Inc. (Provider), with principal offices located at 2809 E. Harmony Road,
Suite 200, Fort Collins, Colorado 80528.
WHEREAS, the above -named parties entered into a contract under which Provider agreed to accept
negotiated rates of reimbursement as payment in full for health care services rendered; and
WHEREAS, Provider is responding to the growing needs of the medical consumers by expanding the
capacity of the medical facilities; and
WHEREAS, the parties desire to include the services provided at Medical Center of the Rockies (tax
identification number, 04-3730045), within the existing Agreement:
THEREFORE:
I. As of the effective date of this Amendment, the agreement entered into by Provider and City of
Fort Collins shall be deemed to amend the other agreement(s) referred to above, such that the terms of
that agreement shall govern the relationship between the parties.
II. All references to Poudre Valley Hospital or Poudre Valley Health Care, Inc. in the Provider
Network Agreement shall be deemed to read Poudre Valley Health System, such that Poudre Valley
Health System is the party in privity of contract with City of Fort Collins.
VIII. In the event of a conflict between this Amendment and any provision of the Agreement, this
Amendment shall prevail. No further modification is made to the Agreement(s) hereby.
IN WITNESS HEREOF, the undersigned duly authorized representatives of the parties have executed
this Amendment.

The rapid population growth of our region will continue to demand additional health care services from
Poudre Valley Health System (PVHS). Space limitations at Poudre Valley Hospital (PVH) preclude
expansion at that location. PVHS is moving some of the PVH services to Medical CenLr of the Rockies
(MCR) in an effort to meet the needs of our patients. The MCR site at I-25 and Highway 34 is in the
very heart of projected population growth. We want to offer health care where the most people are
located and where they can most easily reach the services. We anticipate that the constriction of the
Medical Center of the Rockies and the transfer of designated services will be complete in February of
2007.
MCR is a partnership between Regional West Medical Center in Scottsbluff, Nebraska, and PVHS. It
will be managed by PVHS. I have attached the amendment to your PVHS contract to allow your
members to have seamless access to all of the services they currently receive at PVH. MCR will have a
separate tax identification number (04-3730045) in case you need to enter that information inb your
system. The billing address will remain the same.
