EXHIBIT 3 - CDOT FORM - 1414 - Civil Rights - Anticipated DBE Participation Plan

Exhibit 1 Clarifications
1. Addendum 6 Exhibit 1 Clarification:
• The deleted item should have been listed as “Item 403-34721 - Hot Mix
Asphalt (Grading SX) (75) (PG 58-28)”, this was reflected correctly on the Bid
Schedule.
2. Provided in Exhibits 2 & 3 are the updated CDOT forms 606 & 1414, please use these
documents in lieu of what was provided in the original bid for forms 606 & 1414.
CDOT form 1413 provided with the original bid will still be required with your
submission.
