Service Provider agrees to perform the services identified above and on the attached forms in
accordance with the terms and conditions contained herein and in the Master Agreement between
the parties. In the event of a conflict between or ambiguity in the terms of the Master Agreement
and this Work Order (including the attached forms) the Master Agreement shall control.
The attached forms consisting of four (4) pages are hereby accepted and incorporated herein, by
this reference, and Notice to Proceed is hereby given after all parties have signed this document.

PROJECT DESCRIPTION: Operations Services – HVAC Hydronic System Demo/Install, Phase 1
We are pleased to offer our proposal to complete the requested HVAC hydronic system
demo/install work in the basement east/west wings. It is our understanding the work completed
to-date includes the installation of (6) VAV boxes (1 thru 6), hot water piping mains in east/west
wings and terminal unit hydronic piping for the (6) VAV boxes. The remaining desired work
includes the installation of (3) FCUs (FCU 1,2,4), (4) VAVs (7 thru 10), remaining hydronic branch
piping, temperature control valves for new equipment and hydronic piping insulation for the
entire east/west wings. The demolition of the existing hydronic piping will be completed during a
later phase. A highlighted drawing set has been attached to this proposal to clarify which scope
has yet to be coordinated/completed in the basement east/west wings. The proposal is based
upon a site visit on 12/03/20. Below is our suggested scope of work.

OUR PROPOSAL INCLUDES THE FOLLOWING:
General Construction Activities:
• Provide permit
• Provide new equipment startup and commissioning to ensure proper operation
• Provide one-year warranty on new equipment, materials, and installation craftsmanship
Demolition:
• Provide labor and materials to isolate and drain (3) fan coil units [FCU-1,2,3]
• Provide labor and materials to demo existing (3) fan coil units [FCU-1,2,3]
• Provide labor and materials to recycle the existing equipment per applicable
environmental codes and regulations
New Work:
• Provide labor and materials to core drill as needed for all remaining new hot water piping
paths
• Provide labor and materials to install remaining hot water supply/return piping (Copper,
ProPress fittings) in the east and west wings, with caps for risers going to 1st level and
boiler room
• Provide labor and materials to install temperature control valves for the (3) FCUs, (4) VAV
boxes, and (6) system 2-way valves shown in drawings
• Provide labor and materials to install insulation for all hot water piping in the east and west
wings
OUR PROPOSAL EXCLUDES THE FOLLOWING:
Providing any labor, materials, or equipment for work not explicitly detailed in the above scope,
including but not limited to:
• Labor outside normal business hours, 7:30 a.m. to 4:30 p.m. Monday through Friday
• Engineering
• Electrical
• Controls
• Ceiling Grid
• Drywall painting/patching
• TAB services
• Hazardous material testing and abatement
• Any provision of temporary heating, cooling, ventilation or exhaust
