Gentlemen:
On January 26t", 2018, a representative of Weeks & Associates observed foundation
wall steel at the referenced site. Observations indicate the foundation steel
reinforcement and void form placement is in substantial accordance with the
engineered foundation plan for the project by Weeks & Associates. In addition, the
electrode had been placed in accordance with the recommendations by NEC.

Re: Perimeter Drain & Damproofing Observation

Gentlemen:
On February 5111, 2018, Weeks & Associates observed the perimeter drain at the
referenced site. Observations indicated a 4" diameter perforated pipe had been
placed around the exterior perimeter of the foundation and sloped to the sump pit. The
drainpipe installation is in substantial accordance with the perimeter drain
recommendations for the project. In addition, the exterior of the crawl space
foundation walls had been damproofed.

As requested, on January 23T4, 2018, Earth Engineering Company, Inc. (EEC) personnel provided
construction observation services for the referenced project. On that date, drilled pier foundations
were being constructed for the residence at the referenced address. Construction observation
services were provided by EEC personnel on a part-time basis and included observations of the
pier drilling operation and observation ofpier steel reinforcement. Results of the field observations
completed by EEC personnel during the referenced period are included with this report.
On January 23`d, 2018, EEC personnel observed the drilling operations for the caisson foundations
for the residence under construction at the referenced address. Items observed by EEC personnel
included plumbness of the drilled shafts, stratification of soils and bedrock encountered, caisson
length, cleanliness of the bearing surface and difficulties encountered during excavation. The
materials encountered in the pier borings observed in the garage and basement excavation areas
consisted of brown sandy clay soils underlain by brown sandy claystone bedrock at a depth of
approximately 3 to 7 feet which extended to the depths explored. The piers were drilled a minimum
of 22 feet in length with a minimum penetration of 15 feet into bedrock. The pier hole bottoms
appeared relatively clean with water infiltration observed in some of the piers at the time of our
observations. The drilled shafts observed appeared plumb, were approximately 10 inches in
diameter and extended to a depth of approximately 22 feet at various locations. At the time of our
observations, 3 #6 bars were being placed in each caisson excavation and concrete was being
placed. The observed pier excavations appeared to be in general accordance with the construction
plans prepared by Weeks & Associates, Project No. 217222, dated November 8, 2017.
