(GROUND) performed during the construction of the Banner Health Medical Center Expansion (address: 4700 Lady Moon Drive, Fort Collins, Colorado). The materials testing was performed on a part- time basis at times scheduled by the owner, owner's representative, contractor, and/or subcontractor. During construction, representatives of our office observed reinforcing steel placement and performed testing on the structural concrete placed for the footings, foundation walls, and floor slabs from February 15"' 2017 to April 26"' 2017. The structural concrete was tested to determine physical properties and compressive strength. We performed observations during the installation of the damp proofing and foundation insulation on March 271h, 2017. Nuclear moisture density testing was periodically performed on the structural fill soils and bearing soils during the building excavation and backfill to determine percent compaction and in -place moisture content from January 25t" to. March 27"', 2017. Laboratory testing included the determination of moisture and density relationship and soils classification. A Certified Welding Inspector (CWI) was on -site from April 71h to May 19"', 2017 to observe field welded and bolted connections. Observations were performed during the erection of the structural steel. Based on the test results and to the best of our knowledge, it is our opinion that the materials tested as outlined above for soil compaction, concrete, reinforcing steel, damp proofing, foundation insulation and structural steel welded and bolted connections were in general accordance with project plans and specifications. The materials testing as indicated above was periodically performed to provide data to the Owner, Contractor, and Subcontractors to assist them in evaluating the general compliance of the tested materials and/or work with the contract documents at the times of our observation and testing. We did not supervise, direct or have any control over the Contractor's work. We were not responsible for the Contractor's means, methods, procedures, techniques, or sequences of construction, nor responsible for Contractor to carry out the work in accordance with the contract documents.
