The resident director will Submit license in 30 days, waiting for testing date. -5- FUTURE PROJECTION Future goals are to research and develop a plan to construct a 50 bed facility located in the county of laramier, on a 5 - 7 acre Parcel. Estimated to take approxunately 1 year to build. This facility Will serve all of Colorado. A 10 FINANICAL OBJECTIVE The financial objective of the company is to make sure that the company Remains stable . Being nonprofit, all monies received by the facility will Go to paying off all debts . Any monies left over will be allocated to a Special fund to Compensate for any delay of payment owed by Medicare/Medicaid Or other insurance companies. Funds will be solicited via private and Government foundations . FINANICAL NEEDS AND REQUIREMENTS Initial funding of Ultimate Loving Carewifl be derived from private Funds and contributing foundations. The dollar requirement for Operating the company is projected to be 350,000.00 for the first Twelve months. The money is to be allocated in the following . Manner. 70,000.00'- down payment for the purchase of a home in Dakota ridge 3107 San Luis St. valued at 290,000.00 75,000.00 - working capital and corporate expenses, including travel Expense reimbursement for officers of the company. 10.000.00 - retrofitting the home for patient safety and . bringing it to Fire code. 15000.00 - company debts, expenses, consulting fees.. 45,000.00 - allocated to special fund to compensate for any Delays in payments from all insurance companies. 3505000.00 -3- Other professional staff includes a licensed professional nurse and a Licensed dietician on call. There will be a rotating staff under the Supervision of the licensed nurse. Day shift 6am - 2 pm, consisting of one qualified medication Administration person (QMAP), certified by the state of Colorado, One personal care provider (PCP) trained by licensed medical staff. Evening shift 2 pm - 10 pm consisting of one QMAP and one PCP. Night shift 10 pm - 6 am consisting of one QMAP. All staff will be knowledgeable in body mechanics as well as fire safety. Inseivices will be given periodically to insure current updates. Our home will be subjected to random inspections by the state of Colorado to make sure policies and regulations are being enforced. Under supervision of the charge nurse, charting will be done on a daily Basis as well as monthly assessments to maintain the proper care of The resident and be aware of any physical -or mental change in the Resident. In the event .the resident no longer meets the requirements Set forth by the.facility, they will be transferred to another facility Where they can be properly cared for. BILLING AND CHARGES . , The company proposes to have four residents with the hopes of Expanding to six. Our home or facility will accept Medicare and Medicaid, along with Supplemental insurance also private pay residents. Charges will range between 3500.00 - 4000.00 monthly. The insurance Department will bill Medicare/Medicaid on the first of every month, Private pay residents will be billed on the 15`" of every month. -2- BUSINESS DEFINITION Ultimate Loving Care"being a nonprofit corporation accepts residents By a. Doctor's prescription only provided by their'primary care Physician. Under supervision of licensed staff, the residents are Assessed by the charge nurse to insure that the resident meet the Requirements set forth by the facility. The resident will be alert and oriented to self and surrounding times Three, able to ambulate on own with or without helpful devices or With little or -no assistance from staff, can bathe of shower at least Twice a week with little or no assistance but always having a staff Member to standby assist to prevent a risk of falling. The resident will be able to participate in activities provided by The facility as well as the family members. They are welcome to Come to the dining room for all meals and invited to interact with The other residents. The facility will always be available to family members. Activities Will be provided on a daily basis by the activities department. Three meals plus snacks will be provided daily prepared by Specification and under the supervision of a licensed dietician. _Transportation will be provided the resident for doctor's Appointments and various activities. Our home will be retrofitted to accommodate our residents for Safety according to the state of Colorado and other regulating Agencies. Our staff will consist of a Licensed Resident Director; licensed . By the state of Colorado. In doing So, she found that the quality of care varied. She also realized that The Parkinson's patient was most misunderstood and decided that She would pursue this area of care.
