PVH Harmony Campus Expansion February 28, 2013
The only remaining comment on the utility plans that I have at this time is that note 48 of
the General Notes does need to be included. Just place None after the :. This note helps
us to identify and know when variance have and have not been granted to on the project.
A set of final mylars will need to be submitted for signatures once all comments are
addressed.
The request was routed and I received no concerns regarding the vacation, so once the
existing utilities have been removed and the new waterline accepted I can process the
easement vacation. Once I have the resolution written I can let you know what the filing
fees will be.
The easement vacation request and legal description was received. The legal is being
reviewed and the request will be routed. As for the timing of the easement vacation the
easement cannot actually be vacated until the waterline has been relocated and accepted
in the new location. The waterline relocation work cannot start until the plans are
approved and a Development Construction Permit (DCP) has been issued for the project.
The legals for the three deeds of dedication have been reviewed and look good. To be
able to accept these easement dedications the following is needed:
1. Application and processing fee of $250 per easement. Paid to City of Fort
Collins. You may be able to dedicate all 3 of these with one document, but that
all depends on if the property underlying all of these is the same or not.
2. Deeds provided signed by the owner and the attorney certification signed by the
owners attorney. If you utilize our standard format I do not need to see the deeds
before you get them signed. If there are any modifications made to the standard
format I would recommend that you provide them for City review and approval
before obtaining signatures.
3. I will need a check for the filing fees. I can't determine what this amount will be
until I have all the documents and know how many pages it will be. Once I have
the final documents I can provide you with the amount of the County's filing fees
and will need a check made out to Larimer County Clerk.
The deeds of dedication will need to be received and in acceptable format before I can
sign the utility plan mylars.
With this project any damaged public curb, gutter and sidewalk existing prior to
construction, as well as public streets, sidewalks, curbs and gutters, destroyed, damaged
or removed due to construction of this project along and adjacent to the public streets,
shall be replaced or restored in like kind at the Developer's expense prior to the
acceptance of completed improvements and/or prior to the issuance of the first Certificate
of Occupancy. The limits of the repairs will be identified in the field by the City
Engineering Inspector prior to and over the course of the project and will not be shown
on the plans. We are aware of some areas of damaged sidewalk that will need to be
repaired.
A Development Construction Permit will need to be obtained prior to starting any utility
or grading work on the site. I will send off the application form and information on this.
We can have the preconstruction meeting prior to approval of the plans and minor
amendment, but the permit cannot be issued until after the projects plans and the minor
amendment have been signed off on and approved.

RE: PVHS — Harmony Campus Cancer Center Addition
Dear Peter,
On behalf of Poudre Valley Health System, we wanted to share with you some
background on the Cancer Center Addition project seeking approval by Minor
Amendment. The building addition is the result of nearly a decade of planning and
fundraising toward the development of a much -needed fully functional cancer
treatment center in Fort Collins.
The project was originally to be a stand-alone Cancer Center on undeveloped land
at Harmony Campus. Through the extensive planning phases, the plans were
revised in November 2011 to instead construct an addition to the already existing
Cancer Center of the Rockies facility on Harmony Campus. This existing facility
houses the System's radiation oncology department and was recently remodeled to
make room for a TrueBeam STx linear accelerator, regarded as the world's most
advanced technology for treating cancer and other potentially life -threatening
illnesses. By making this relocation adjustment for the Cancer Center, PVHS is able
to leverage these recent technological advances to develop a comprehensive center
for the community with greater efficiencies and cost savings.
The design process for this center has been extensive. In 2012, PVHS held three
week-long sessions involving 25 physicians and cancer patients to gain their ideas
for how to best meet the needs of patients. The physicians were from various
disciplines to help ensure that they designed a fully functional center for the
betterment of all patients and building occupants. During the fundraising and
development stages of the Cancer Center over the last seven years, PVH has issued
press releases offering updates. These news announcements were reported
through the local newspapers, television and radio, as well as on VIC's news page
and ProviderLink, their intranet for physicians. In addition, several widely publicized
fundraisers were held during those years. They also placed information on their

It looks like one of the new Fire hydrants (the one closest to the building) is not actually
located within the existing utility easement. If the proposed Fire hydrant is to remain in
this location dedication of a utility easement will be needed prior to approval of the plans.
I can provide you will base dedication documents and information on this process if you
need it.
The new sewer line appears to be crossing two areas in which utility easements do not
exist. Dedication of utility easements for these areas is needed prior to approval of the
plans. I can provide you with base dedication language and information on this process if
you need it.

Need to make sure that it is clear that the curb, gutter, sidewalk and ramp details provided
on the plans is for on -site use only and does not apply to any work to be done on the
public sidewalk system or streets.

Even with this continued outreach, some physicians at Harmony Campus have
expressed to PVH that they weren't fully informed of the project. As a result, PVH
held a public design review meeting on October 31, 2012 and a public owner's
meeting on November 27, 2012 to discuss key project issues. During these
meetings, site and parking access was a key consideration. As a result, PVH
commissioned a parking study through an outside consultant. The findings support
the addition, confirm that the amount of parking spaces is adequate, and make
recommendations for prompt snow removal so that existing parking remains fully
usable to lessen the perception of limited parking. PVH met again with the Harmony
Campus owners last week on January 3 to present and further discuss the parking
study. PVH is following up with a snow removal plan as well as a parking plan that
would define employee parking areas leaving up -close parking for patients and
visitors. They also plan to complete another parking study once the project begins
construction and the parking spaces are removed to monitor the effects and make
any resulting adjustment to parking assignments or maintenance practices.
PVH plans to continue to work closely with physicians and tenants at the Harmony
Campus to ensure its continued success as a key part of healthcare delivery in Fort
Collins. If you would like to view the parking study or any additional information

You are correct. There is an existing water main which crosses the area included in this easement vacation; therefore,
the easement cannot be vacated until the new water line is installed, tested and accepted by the City.

there is an existing waterline running through the area and if agreed to, the vacation will not occur until after the
waterline has been relocated out of the area to be vacated.

easement and/ or if there are any other utilities that will need to be relocated prior to the vacation occurring. Please
