Work Description: NEW 16,216 SQ. FT., 2 STORY ADDITION FOR THE SOUTH EXPANSION TO THE
EXISTING AVAGO BUILDING #4 WEST ANNEX (QUAD 5). THE PERMIT WILL INCLUDE
8,000 SQ. FT. OF INTERIOR FINISHES OF CLEAN ROOM SPACE ON THE SECOND FLOOR
IN THE NEW ADDITION, AND MODIFICATIONS TO THE EXISTING EGRESS CORRIDOR IN
BUILDING 4 (THE CORRIDOR WILL LEAD TO THE NEW ADDITION). STRUCTURAL STEEL
FRAME CONSTRUCTION. F&F ISSUED ON 12/10/2014
This is to certify that the building, structure, or land located at 4380 ZIEGLER RD, BLDG 4 Fort Collins, CO has
been inspected and temporary occupancy is hereby authorized, effective on 07/20/2015 and conditional upon the
following requirements being completed and approved upon final inspection.

0720/2015 Still need elevator special inspection report. 3. Special inspection required on elevator & structural masonry.6. Building air
tightness test required.
Fire Authority Final -Requirements to be Completed By: 09/30/201&

06v18/2015 Ok for 90 days.
If such work is not completed within the.time set forth above and approved on final inspection, or unless the
time for completion is extended by the Building and Zoning Office, an order to vacate the premises, or a
violation notice for occupying the building, structure, or land without a Certificate of Occupancy will be
issued. Such violation is subject to a maximum fine of $1000.00 per day for every day of violation.

NOTE: The issuance of this temporary certificate of occupancy will entitle the owner to place City Utility service in the owner's name. Construction power rates
may be substantially higher than the rate to which the owner is entitled. Until the owner contacts the City Utility Office and makes the necessary arrangements to
transfer City Utilities to the owner's name, utilities will continue to be furnished at construction rates and both the owner of the property and the contractor will
continue to be liable to the City for all utility bills.
