HEALTH MEDICAL CAMPUS. BUILDING ELEVATIONS, LIGHTING AND SITE
IMPROVEMENTS MUST COMPLY WITH APPROVED DEVELOPMENT PLAN.
This is to certify that the building, structure, or land located at 4700 LADY MOON DR Fort Collins, CO has been
inspected and temporary occupancy is hereby authorized, effective on 01/27/2015 and conditional upon the
following requirements being completed and approved upon final inspection.
Bldg Dept Req Docs Rcvd Requirements to be Completed By:
01272015 Received elevator inspection letter.
Need: Blower door, med-gas inspection, commissioning report.
Engineering Final.. Requirements to be Completed By: 4/30/2015 .
0126/2015 TCO is good for 90 day we will call it April 30th
WWW Irrigation Final Requirements to be Completed By: 05/15/2015 .
:.01222015 Installation Site Inspection: Contact Eric Olson at 970-221-6704 or eolson@fcgov.corh when irrigation construction begins. .
Utilities. will perform periodic site inspections to ensure installation follows the approved plan.
Zoning Final Requirements to be Completed By: 03/31/2015
01272015 Landscape bond reed CO pending the completion of RTU/vent/equipment screening, bike parking, handicap parking signage,
removal of temporary signs/construction debris,-port-o-lets.
Building Insp Completed Requirements to be Completed By:
01272015 OKfor 45 days.
Chapel incomplete, glazing missing & S. stair tower handrail is not installed.
If such work is not completed within the time set forth above and approved on final inspection, or unless the
time for completion is extended by the Building and Zoning Office, an order to vacate the premises, or a

NOTE: The issuance of this temporary certificate of occupancy will entitle the owner to place City Utility service in the owner's name. Construction power rates
may be substantially higher than the rate to which the owner is entitled. Until the owner contacts the City Utility Office and makes the necessary arrangements to
transfer City Utilities to the owner's name, utilities will continue to be furnished at construction rates and both the owner of the property and the contractor will
continue to be liable to the City for all utility bills.

violation notice for occupying the building, structure, or land without a Certificate of Occupancy will be
issued. Such violation is subject to a maximum fine of $1000.00 per day for every day of violation.
