Dear Mr. Jernigan:
Please fund attached the comprehensive asbestos survey performed on the above referenced facility.
I, licensed through the Colorado Department of Public Health and Environment.. for inspections.
CDPHf: Certification No. 12400, performed this survey on September 23, 2011. All samples were
analyzed by Polarized Light Microscopy (PLM) in ourNV.LAP accredited laboratory (NVLAP Lab
Code 4101232-0), and confirmed to be non asbestos -containing.
Current EPA and State of Colorado regulations require that an asbestos survey be performed prior
to disturbing building materials (demolition or renovation) in a public building. The State of
Colorado regulations (adopted NESHAP regulations) require that a notification be filed with the
Colorado Department off Public Health and Environment (CDPFIE) only if structural members are
removed/demolished, or regulated asbestos -containing materials (.1ZACM greater than 260 l.f , 160
s.f. or 35 cu.0 are disturbed.
The State of Colorado regulations require that a copy of the asbestos survey be kept can the job site
at all times duringrenovationactivities. The attached report will satisfy this requirement.
In summary, no confirmed asbestos -containing materials were found Sam's Club Store 46633,
4700 Boardwalk Drive, Ft. Collins, Colorado. No further action with regard to asbestos -
containing materials is required for this store. An invoice for our services is enclosed. We have

Laboratory Analysis Report
This section contains the laboratory analysis report documenting the laboratory's findings, analytical

Asbestos laboratory reports identify samples for which no asbestos was detected are indicated by

Improved.Method for the Determination of Asbestos in Bulk Building materials. 40 CFR Part 763, Subpart E,
Appendix I, interim method varies slightly from this method. Quantification is by visual estimation. In accordance
with the EPA revisions 40 C:FR Part 61, pertaining to friable materials, if the asbestos content is estimated to be less
than 10 percent by visual estimation, the detennination may be repeated upon request, using the point counting
technique. NOTE: Asbestos fibers may occur in particle size below the resolution limit of this analysis
technique; TEM analysis is recommended on samples of floor tile determined to be <1 % or none detected by
this method. The percentage of asbestos reported refers to the overall percentage for the material provided, unless
otherwise indicated. Samples determined to be <1 % by visual estimation are automatically repeated using the point
count technique. Samples are analyzed by layer but results may be combined if no regulated fibers were observed.
RESTRICTIONS: This report relates only to the items actually tested, and must not be used to claim product endorsement
by NVLAP, or any agency of the United States government. Reproductions must include the entire report.
RESULTS: The percentage of asbestos reported is by visual estimation. Point Counting results, when appropriate, are
reported in parenthesis after these results in the Sample .Description.

I. PROJECT OVERVIEW
ERI Consulting, Inc. of Tyler, Texas, was commissioned by Mr. Shannon Jernigan, of Wal-
Mart Stores, Inc. #8085 to conduct an asbestos inspection of the Sam's Club Store #6633,
4700 Boardwalk Drive, Ft. Collins, Colorado. The inspection was performed in accordance
with applicable CDPHE, OSHA, and EPA standards. The purpose of the inspection was to
determine the location of the asbestos -containing materials used in the construction of the
facility. The inspection was performed by Mr. Christopher L. Power, licensed through the
Colorado Department of Public Health and Environment, for inspections, CDPHE
Certification No. 12400.
The field inspection was performed on September 23, 2011. During the inspection the
facility was occupied. Access to all areas of the building was provided by store manager.
The inspection involved the collection of 69 bulk samples, representing 23 homogeneous
materials.
The initial phase of the inspection involved the identification of the homogeneous areas
suspected of containing asbestos fiber. Each homogeneous area was quantified to determine
the approximate amount of suspect materials present. These areas were then divided into
sectors and samples collected in a statistically random manner. A sufficient number of
samples were collected in each homogeneous area based on the amount of suspect material
present.
All samples collected during the inspection were delivered to the laboratory of ERI
Analytical immediately following the field inspection. Suspect asbestos -containing samples
were analyzed by Polarized Light Microscopy (PLM), in accordance with 40 CFR 763
Subpart E Appendix E. This report contains information documenting the field inspection
and laboratory findings.
The following building materials were identified as non asbestos -containing in the field and
were not sampled.
Fiberglass and/or foam insulations on water lines and fittings
Insulation on various HVAC ducts
Rubber HVAC expansion joints
Ceramic tiles throughout kitchen areas
Limitations: Limited destructive sampling was performed as part of this inspection;
therefore, there may be other materials located throughout the building which contain
asbestos but were not sampled. Such materials, if present, would be located in enclosed or
concealed pipe chases, behind walls, or other inaccessible areas. Such materials would only
be encountered during extensive renovation and/or demolition activities.

II. HOMOGENEOUS AREAS
The following homogeneous areas represent the suspect materials which were sampled
during the inspection to determine their asbestos content. The definition of a homogeneous
area, as used in this report, is a building material that is uniform in style, appearance, color
and/or texture.
SUSPECTED ASBESTOS -CONTAINING MATERIALS
Floor Coverings
Area A - Gray flooring "A" - located throughout various areas, including the deli,
rotisserie, and bakery areas.
Area B - Red flooring - located throughout the meats area.
Area C - Yellow flooring - located throughout the exit areas.
Area D - Gray flooring "B" - located throughout various areas, including the
registers, clothes, and freezer areas.
Area E - Floor caulk - located throughout various areas, including the registers,
clothes, and freezer areas.
Area F - Carpet with glue - located throughout the pharmacy, optical show room,
and optical office areas.
Wall Materials
Area G - Texture wall surfacing material - adhered to the sheetrock wall material
throughout the building.
Area H - Sheetrock wall material - located throughout various interior walls of the
building.
Area I - Tape and bedding joint compound - used to join the sheetrock wall
material throughout the building.
Area J - 4' x 8' wall panel - located throughout the automotive restroom, deli, and
produce preparation areas.
Area K - Expansion joints -located throughout the perimeter walls.

Area L - Texture ceiling surfacing material - adhered to the sheetrock ceiling
material throughout the building.

Area O - 2' x 4' lay -in ceiling tile "A" - located throughout various areas, including
the deli, bakery, and rotisserie areas.
Area P - 2' x 4' lay -in ceiling tile "B" - located throughout various areas, including
the restroom common area, mezzanine office, and dock office areas.
Area Q - 2' x 4' lay -in ceiling tile "C" - located throughout the produce preparation
area.
Thermal System Insulation
Area R - TSI white putty - located in the mechanical room, dock, and optical areas.
Area S - Gray HVAC duct mastic - located in the mechanical room, dock, and
optical areas.
Miscellaneous
Area T - Black base cove with glue - located throughout various areas of the
building, including the customer service, lounge, and dock office areas.
Area U - Plaster material - located on the exterior front overhangs/fascias of the
store.
Area V - Cinder block - located on the exterior of the building.
Area W - Cinder block mortar - located on the exterior of the building.

The following is a summary of the confirmed asbestos -containing materials found at the
Sam's Club Store #6633, 4700 Boardwalk Drive, Ft. Collins, Colorado. In accordance with
Section 763.85 ofthe Asbestos Hazard Emergency Response Act, all suspect materials have
been listed by homogeneous areas.
