Dear Mr. Everitt:
On October 9, 2008 this office observed and approved the continuous spread footing form
dimensions (widths and depths) and the interior square footing form dimensions (widths and
depths) at the location referenced above. The size, type and installation of the continuous spread
footing and the interior square footing reinforcement is approved. At the time of this
observation, the #4 hooked dowel reinforcement was on site and it is to be installed in the freshly
placed footing concrete. The dowel installation and spacing is to be verified when the
foundation wall reinforcement is observed.
The continuous footing form dimensions and reinforcement and the interior footing form
dimensions and reinforcement are consistent with the foundation design by Axiom Structural
Engineering, LLC, project number: JAMB-08G03-24-25, dated July 9, 2008.

Dear Mr. Everitt:
On October 15, 2008 this office observed and approved the horizontal and vertical reinforcement
placed in the foundation wall forms at the location referenced above. The reinforcement
observed meets the specifications for size, type and installation in accordance with the
foundation design drawings by Axiom Structural Engineering, LLC, project number: JAMB-
08G03-24-25, dated July 9, 2008.
We also observed the #4 rebar `Ufer' ground rod that projects vertically from the top of the
exterior Northeast corner of the basement foundation wall. The `Ufer' ground rod is lapped
approximately 3'-0" and it is tied to the bottom row of the continuous horizontal foundation wall
reinforcement. The vertical projection of the `Ufer' ground rod is located approximately 5'-8"
along the East foundation wall from the Northeast foundation corner as described above.
The strap -tie anchors were observed on the site and are to be located in the foundation walls as
indicated on the plan referenced above.

Dear Mr. Everitt:
On October 21, 2008 this office observed the foundation sub -drain materials installed around the
exterior perimeter of the basement foundation. The sub -drain pipe is connected to a sump pit in
the basement. The homeowner should check the sump pit periodically for water. If water
accumulates in the pit, a licensed plumber and electrician should install a submersible pump with
an automatic float switch. The pipe from the pit should discharge at an acceptable location away
from all foundation walls, window wells and exterior concrete flatwork. At the time of this
observation the foundation sub -drain materials appeared to be properly installed.
We also observed the spray -on damp proofing applied to the exterior of the basement foundation
walls. The damp proofing appears to adequately cover the basement foundation walls. Please
note that spray on damp proofing is typically effective for reducing moisture infiltration in the
absence of hydrostatic pressure. Spray on damp proofing does not create a waterproof barrier
that is capable of resisting moisture infiltration under hydrostatic pressure. Thus it may be
possible for water to infiltrate the foundation, particularly at form -tie penetrations and hairline
cracks that sometimes occur from shrinkage, especially near basement window openings. It is
important to provide and maintain drainage paths that allow all surface water to flow quickly
away from all foundation walls, window wells and exterior concrete flatwork.
