Remarks or Deficiencies Noted: The sheathing at the live and dead ends of some tendons was not per PTI standards. The dead-end of one tendon lacked proper clear cover due to a loose form. Various tendons lacked proper clear cover from sub -grade. All corrections were made while a representative from our office remained on -site.
