0 Reinforcement was placed in general conformance with approved drawings and standard construction practice.
Remarks or Deficiencies Noted:
The sheathing at the live and dead ends of some tendons was not per PTI standards. The dead-end of one tendon lacked proper clear cover
due to a loose form. Various tendons lacked proper clear cover from sub -grade. All corrections were made while a representative from our
office remained on -site. Mike Gordon of M&R Concrete was infbr��at the top of the trenches would need to be cleared of debris prior
to Dour.
