FIELD OBSERVATION REPORT

Casey's Car Care Center
4315 South Mason
Fort Collins, Colorado 80526
This report is to record my observations of the foundations for the project prior to placement of
concrete.
In general, the reinforcing was in compliance and placed as shown on the drawings. Minor
adjustments were necessary and the field personnel were so advised. The corrections were made
on the spot and the pour could proceed.

This report is to record my observations of the structure for the project prior to completion.
In general, the installation was in compliance with the structural drawings.
The following corrections were required and the field personnel so advised on two separate
occasions.
1. Where the gists were moved, or where holes were torched through, washers shall be
placed in all instances.
2. Where the man -door is framed in)the vertical member shall be continuous between
horizontal members.
