To Whom It May Concern:
On July 28 and 29, 1997, our home was damaged due to sewer back-up as a result of a flash
flood in the Fort Collins area. Damage occurred in the basement of our home at 821 Mathews,
Fort Collins, Colorado 80524 and consisted of the following:
floor covering;
cabinetry; and
sheet rock, up to 3 feet in height from the floor.
The City of Fort Collins Building Permit Department has stated to us on September 22, 1997,
that a building permit will not be required to replace and repair the above.
