Gentlemen:
We have visually observed the wall reinforcing and void material placed in the forms at the residence
under construction at Lot 52, Shenandoah, (6903 Lyeena Court), a subdivision of Fort Collins,
Colorado. This observation was performed on July 20, 1999.
It is our opinion that the reinforcement was in general conformance with MNA, Inc. foundation plan,
Job No. 1024.

Gentlemen:
We have visually observed the footing forms placed at the residence under construction at Lot 52,
Shenandoah, (6903 Lyeena Court), a subdivision of Fort Collins, Colorado. This observation was
performed on July 16, 1999.
It is our opinion that the form widths are insubstantial conformance with MNA, Inc, foundation plan,
Job No. 1024. No substantial amounts of loose material were noted below footing, wall or pad
bearing locations.
This observation does not, nor was it intended to cover setback requirements. It is the
contractors/owners responsibility to verify that the home has been located within the appropriate
building setbacks.
