Gentlemen On November 29, 1999 Weeks & Associates observed the foundation wall reinforcing and void
form placement at the referenced site. Observations indicate the reinforcing steel and void
form were in substantial conformance with the foundation plan for the site by Weeks &
Associates, Inc., Project No. 99276 dated November 8, 1999, revised November 16, 1999.

On November 17, 1999 Weeks & Associates observed the drilled pier installation at the
referenced site. Observations of piers at the east side of the basement indicate the piers
observed were installed in substantial conformance with the foundation plan for the site by
Weeks & Associates, Inc., Project No. 99272 dated November 8, 1999, revised November 16,
1999.
