Empire Laboratories, Inc., observed the foundation construction on November
11, 1988, a residence being constructed at 4213 Beaver Creek, Fort Collins,
a site more particularly described as Lot 38, Block 7, Willow Park, Fort
Collins, Colorado.
Observations made during the foundation construction indicate that the found-
ation wall steel placement is in accordance with the plans and specifica-
tions for this project.
