SUBJECT: Inspection.of Caisson Drilling for Lot 1, Harmony Ridge,
City of Fort Collins, Larimer County, Colorado
CDS Project No. 12-6271
REFERENCED FOUNDATION PLAN: CDS Project No. 12-6271
On May 6, 2013, we visually observed the drilling operation of thirteen (13) of the twelve
(12) inch diameter caissons for the residence under construction at the above referenced site. The
caissons were located in the basement of the proposed residence. These caissons were drilled to a
minimum depth of thirty-three (33) feet with a minimum penetration of twenty-six (26) feet into
competent bedrock. The caisson holes were relatively clean and groundwater was present.
The caissons were drilled in general conformance with the above referenced foundation
plans. Three (3) #5 reinforcement bars were placed in the caisson holes prior to the placement of
concrete. Concrete was placed by means of a pump truck.
This operation was found to be in accordance with good construction practice and in
accordance with the engineer's specifications.

RE: Observation of Reinforcing Steel at Lot 1, Harmony Ridge,
City of Fort Collins, Larimer County, Colorado
CDS Project No. 13-6271
Dear Fran,
On May 17, 2013, we visually observed the steel reinforcement in the wall forms for the
residence under construction at the above referenced site. The reinforcement appears to be in
general conformance with the foundation plan by CDS Engineering Corporation, Project No. 13-
6271, dated February 5, 2013. We also observed that a UFER ground was installed.

RE: Observation of Perimeter Drain at Lot 1, Harmony Ridge,
City of Fort Collins, Latimer County, Colorado
CDS Project No. 13-6271
Dear Fran,
On May 23, 2013, we observed the partially constructed exterior perimeter drain system and
damp proofing at the above referenced site.
The drain system consisted of corrugated flexible polyethylene pipe surrounded by gravel. The
drain appeared to be located at to below the top of pier level and is to discharge into an interior sump pit.
In addition, it appeared that the damp proofing adequately covered the foundation wall.
As a result of our observations, the perimeter drain system appeared to be in general conformance
with the engineer's recommendations.

RE: Structural Floor for Residence at Lot 1, Harmony Ridge,
City of Fort Collins, Larimer County, Colorado

Dear Fran,
On July 12, 2013 we observed portions of the structural wood floor for the above

referenced residence. The floor appears to be constructed per our recommendations; however,
the beam saddle for the front beam that is attached to the exterior foundation wall, needs one
additional %" Titen HD bolt installed.
The foundation plan for the above referenced residence called out 2 x 12's for the back
deck beam. It should have shown 2 — 2 x 12's.

RE: Bolt installation for the Structural Floor and the deck beam connection for the Residence
at Lot 1, Harmony Ridge Subdivision, City of Fort Collins, Larimer County, Colorado
CDS Project No. 13-6271
Dear Fran,
On July 25, 2013, we observed the installation of the additional Titen HD bolt for the
beam saddle for the W 1 Ox 12 steel beam on the south wall of the house. The bolt appeared to be
installed per Simpson's recommendations.
The attachments of the beam to the post at each deck post was observed, there appeared
to be a minimum of three 5/8" bolts attaching the beam to the post at each post. If there was a
splice in the deck beam, three 5/8" bolts are needed to connect the beam to the post. This
appeared to have been done.
