I. Connection to the sanitary sewer collection system cannot be made until System Development charee is paid to the
District:

service line to main connection. and backfill as required by the District. Federal and State OSHA requircinents, and
applicable Building and Plumbing codes:

Roxelder Sanitation District is not responsible Im the condition of the sewer lateral stub out lines. The properfy
owner is responsible for the condition of these lines and is advised to check for stoppages in said line prior to
connection (i.e. probe and light_ etc.). The District responsibility for the collection system does not extend beyond
the District collector lines within the subdivision. Please be advised that failure to obtain District signatures on this
form cotdd restill in deni,tl of access to the sanitary sewer systcm.
