Enclosed are in -place field density test results obtained by our
personnel after placement in the areas identified at the above -
referenced project.
The field density test results were obtained utilizing a nuclear
moisture density gauge are included, along with the maximum density
and optimum moisture data developed for the material being placed.
The tests were performed in general accordance with the test
procedures referenced on the attached reports.

An engineer from Empire Laboratories, Inc. observed the roof
construction for the above -referenced project on May 4 and 9, 1995.
Based on our observations, it is our opinion the roof deck weld and
screw connections and the steel joist support veld sizes are in
accordance with the plans prepared for the project.

An engineer from Empire Laboratories. Inc. observed the roof beam connection at the four
entry columns at the north side of the building for the above referenced project on June 13,
1995.
The steel roof beam was welded to the bearing plate at the four columns with a 1/4" fillet
weld the full length of the base plate at each side of the steel beam bottom flange. Based on
our observations, it is our opinion the weld sizes are in accordance with the shop drawings
prepared for the project.
