The following items were not clearly specified in the plans or were not in compliance with the Colorado Retail Food Establishment Rules and Reaulations. These items must be clearly specified in writing or be modified to be in compliance with the above regulations prior to approval of the submitted plans. The plans are APPROVED at this time. This letter is also in response to the July 14, 1999 plan review letter. I) Proposed menu: Popcorn, soft drinks (ice), ice cream novelties, hot dogs, and nachos 2) Submit information on proposed food handling procedures for the following areas: Please include any Hazard Analysis Critical ControlPoint(HACCP)-Plan/Food Handling- Procedure- Manual -that _details the -following: - a) Protocols and/or systems that will be set up to monitor food temperatures. b) Policies to restrict or exclude food workers who are ill or have infected cuts or sores. c) Policies/procedures to reduce or eliminate the handling of ready to eat foods. d) Information detailing what foods will be dispensed in bulk and what equipment will be used to dispense these foods. 5) Heavy equipment (e.g.: reach in refrigerator, reach in freezer, and popcom popper) must be mounted on casters to allow for mobility and easy cleaning. Be advised heavy counter mounted equipment (e.g.: ice cream freezer) which can not be easily moved for cleaning must be provided on four inch approved legs to facilitate easy cleaning. 11) Restrooms must be provided with mechanical ventilation to the outside air. Plans indicate that all restroom are vented to the outside air. Main restroom will vent 1300 cfm, left side restroom 700 cfm. APPROVED 14) Please indicate if a garbage disposal will be attached to any food preparation or warewashing sinks in this establishment. Resubmitted plans indicate that NO sinks will have garbage disposals. 17) A water heater, for the concession stand only, that can achieve a minimum recovery rate of 113 gallons/hour if gas, or 94 gallons/hour if electric, or has a 125,000 BTU rating or has a 23 kW (kilowatt) rating is required. The Lochinvar water heater, model HC 27-082 ( 110 gallons /hour) is APPROVED. 18) All drain lines from equipment such as: 3-compartment warewashing sinks, food preparation sinks, ice machines, ice bins, drink dispensers, drink overflow drains, refrigeration/freezer units not provided with evaporation pans, water heaters and cold tables holding ice must be directed to a floor drain or floor sink and be properly air gapped. Cinemark Page 2 20) Please indicate if chemical -dispensing towers will be provided. if so please indicate where they will be located and how they will be plumbed. If provided, chemical towers must be supplied with their own water lines. Resubmitted plans, from Urban Architecture indicate that there is a service sink for the chemical tower. Indicate if the service sink is separate from the mop faucet. 34) Toxic materials and surplus cleaning items must be stored away from food operations, food storage and utensil washing facilities. Plans -indicate -that -chemicals willbestoredawayfrom-food-in-either room -I-lOj 145, or 150. 35) Provide an approved area for the proper storage of employee's personal items, such as: coats, clothing, etc. Employee items will be stored in the break room away from food. 36) Please be advised that the following items are required in the establishment at the time of the opening inspection and thereafter: a) stab food thermometer (0 to 220 F) b) food grade plastic gloves and/or finger cots c) approved sanitizer d) test strips for the sanitizer e) copy of state sales tax license 37) The Latimer County Department of Health and Environment must receive a completed Retail Food Establishment License Application, State Sales Tax License number, FEIN number, and all applicable fees before an opening inspection will be scheduled. The written plans submitted are approved at this time. Please submit information on items2, 20. Be advised approval of plans, including the above modifications does not constitute acceptance of the completed structure. This department prior to approval for operation and licensure of the establishment must conduct an inspection of the premises. After 45 days, any approval of the plans will be null and void and plans will have be resubmitted along with a $75.00 plan review fee for a second review. Please notify the department at least 48 hours prior to your opening date so that a final acceptance inspection can be performed. This department's written approval and the receipt of a completed state foodservice license application and applicable fees are necessary prior to opening. Sincerely, r mas R.
