On August 31, 2000 I performed -&-foundation inspection of the rebar in thespread foe�ing for the car wash and
determined that it met with the intent of the construction documents.
On September 1, 2000 I performed a foundation inspection of the-rebar in the stem wall for the car wash and
determined that it met with the intent of the construction documents.

OBSERVATIONS
In general, the concrete forms were properly set and free of debris. The reinforcing steel required
for the wall footings and integral pads was correctly placed, and the size of the footings and pads
were consistent with the requirements of the foundation plan prepared by this office. Vertical
dowels consisting_ of No. 4 Grade 60 reinforcing_ steel was also ready for placement.

None.

RE: Canopy Footing Inspection

Dear Dave,
On September 7, 2000 I performed -a - foundation -inspection of the rebar for the fuel island canopy footings and
determined that it met with the intent of the canopy footing design drawings prepared by Arning Industries.
Footing -holes were 3' V diameter x 8-'-6' deep. - Footing rebar cages -were 307 diameter x 8' 0" long with (10)
#6 bars vertical with 6" hooks, (3) #3 ties at 2 '/2" o.c. at the top and (8) #3 ties at 12" o.c. to bottom of cage.

Dear Dave,
On September 28' 2000 I performed an inspection of the first 48' (vertical) of structural masonry block for the
car wash and determined that the block, rebar and grout installations met with the intent of the Construction
Documents.
On September 29, 20001 performed -an inspection of the second-48" (vertical) of structural masonry block for
the car wash and determined that the block, rebar and grout installations met with the intent of the Construction
Documents.
On October 5, 2000 I performed an inspection of the structuraFinasomy block for-the-qar wash from 8'-0" a.f f.
to the top of the bond beam and determined that the block, rebar and grout installations met with the intent of the
Construction Documents.
