Collins, CO $DD13gZ'Z� Dear Mr. Amir JUL 2 8 2000 This department has reviewed the plans and specifications submitted for the construction of Chevy's Fresh Mex located in the Harmony Village at 4627 S. Timberline Rd, Ft. Collins, Colorado. The following items were not clearly specified in the plans or were not in compliance with the Colorado Retail Food Establishment Rules and Regulations. These items must be clearly specified in writing or be modified to be in compliance with the above regulations prior to approval of the submitted plans. The plans are approved at this time. All plans must be approved prior to commencement of construction. Please be advised a plan review fee of $30.00 per hour will be charged for time spent reviewing the plans. This fee must be paid before an opening inspection can be scheduled. 1. Submit information on proposed food handling procedures for the following area: Please include any Hazard Analysis Critical Control Point (HACCP) Plan/Food Handling Procedure Manual that details the following: a) Provide a name and number for an individual with whom a menu review and a review of food safety procedures can be conducted b) Protocols and/or systems that will be set up to monitor food temperatures. c) Policies to restrict or exclude food workers who are ill or have infected cuts or sores. d) Policies/procedures to minimize the handling of ready to eat foods. NOTE: All reference numbers used here are from the Equipment Specifications book submitted and not from the submitted blue prints. 2. All food service equipment must be of commercial design, that is certified or classified for sanitation by an American National Standards Institute ANSI accredited certification program or its equivalent. Used equipment and custom made equipment such as the cutting station (200) and the chef s counter (300) cannot be approved until it is visually inspected. The Excelsior margarita barrels (23) are not approved at this time due to lack of sanitation listing information. Please submit proof of sanitation listing or this item will have to be visually inspected for approval.
