1. The purpose of this site visit was to observe the progress of the steel erection.
2. The steel columns, beams and roof bar joists were installed. Workmen were installing the bridging in the bar
joists at the time. The beam to beam and the column to beam connections appeared to be in compliance with the
contract documents.
3. The miscellaneous steel bracing, edge angles and steel roof deck were not installed yet.
4. The repair to the beam connection described on the previous field report, item number one, was repaired in a
satisfactory manner.
5. Another field observation should be scheduled when the miscellaneous steel bracing, edge angles and roof deck
are installed.

The purpose of this site visit was to observe the progress of the steel erection.
2. The steel deck was installed and properly attached to the roof joists.
3. The masons were building the mechanical screen wall on the roof. The appropriate grade 706 rebar was in place.
The strut to support the masonry wall will be installed after the masonry wall is built as shown on sections two
and three on sheet S-2.
4. Most of the miscellaneous steel bracing was installed. At grid four and C and at grid D and four bottom beam
bracing needs to be installed. Andy said that this would be installed when the welders come out and install the
braces for the masonry wall.
5. The main skylight on grid four was built with a continuous six-inch by sixteen gauge stud attached to the bottom
of the bar joist and attached to the vertical studs. With the bar joists having bridging at being x-braced at these
locations it is not necessary to install the 3 5/8xl6 gauge stud kickers as shown in section 5 S-3. The steel frames
as detailed in section 4 S-2 for roof openings such as, roof drains, mechanical units and skylights were in place.
6. Andy said the roofing was going to be stocked and installed on the roof next week.

JVA was the Structural Engineer of Record for this project which was constructed by Drahota
Construction beginning in August of 1996.
During the construction of structural portions of the project, our office made periodic trips to the
site to observe the progress of the structural work. Enclosed are copies of our field observations
from those site visits.
Based on the construction that we observed during those visits, it is our opinion that the
construction was done in general conformance with the requirements of the contract documents.

1. Footing excavation was completed and forms were in place for perimeter footings. Horizontal
footing reinforcing and corer bars were all in place and wired together. Critical dowels for
masonry reinforcing were in place over approximately 50% of the building perimeter. These
dowels are being wired to the horizontal bars and supported with a continuous wood piece down
the center of the footings. Andy indicated that the spacing of these bars had been coordinated with
the mason earlier this morning.
2. Bars were well tied in place with the exception of a few areas where bottom reinforcing was too
close to the exposed earth. Andy and I discussed the need to get these bars pulled up so that they
are three inches clear to the soil.
3. Andy asked about the possibility of cutting off the vertical masonry reinforcing bars to simplify
the block lane process for the mason. We discussed various options to simplify the masonry. We
concluded that there are three ways to solve this problem. A) The vertical bars may be cut three
feet above the top of the footing. If this option is selected, then an additional piece of No. 5 bar
x 6' long would need to be spliced on to the verticals to get the masonry reinforcing back up to the
height that it needs to be. B) The masons could leave the vertical dowels as is and slot the end web
of the pieces of block that had reinforcing in them. C) The masons could put planking across the
width of the excavated trench to make is easier for them to lift the block over the tops of the
vertical dowels.
4. From a structural viewpoint, any one of the three preceding alternatives is acceptable. Andy will
discuss these with the mason and select the solution that will help the mason the best.

5. I verified that the reinforcing was of the proper grade and size. I also looked at the forms and the
mats of reinforcing steel that were tied for the interior footings.. These were not yet down in the
bottom of the excavation, but that will go quite quickly. The concrete pour is scheduled for
approximately 3:00 this afternoon.

The following items were observed:
1. Concrete block foundation walls have been placed up six courses above the top of the footing. Grout had been
poured in the vertical grouted cells in the bond beam up to this level' just a few minutes before I arrived at the
job site.°
2. The masons chose to cut off the vertical dowels coming out of the footing at an elevation approximately three feet
above the top of the footing. The solution to this was to lap vertical no. five bars of each of the dowels.
Unfortunately, the vertical lap bars were not installed prior to grout placement. In a few places, I found vertical
bars that were not well bonded to the grout. Andy and I talked with the masonry superintendent about the
importance of getting these bars vibrated into place by hammering on the bars and by hammering on the external
wall of these reinforced cells. The grout was setting up suprisingly fast caused this problem. I emphasized that
on future grout pours it will be important to have the vertical bars all in place prior to pouring grout.
3. There was no vibrator present at the site to be used in consolidating the grout. This is specifically required by
the drawings and needs to be done on future grout pours.
4. Vertical bats are properly centered in cores and proper comer bars were provided at the comers of all masonry
work. Durawall reinforcement was in place at 16" on center.
5. Andy and I reviewed these issues and the general notes regarding vibrating and providing bar locators. These
items will be corrected before the next grout pour tomorrow or Monday.
6. 1 asked about mortar and grout prisms because none were visible on the job site. Andy said that the testing agency
(Terracon) had requested that the prisms be built at their testing lab to avoid having to ship them. That is why
none of the prisms were at the job site.

The following items were observed
1. I met with Andy and Jack (the masonry superintendent) regarding a problem that has occurredwith misplaced
anchor bolts at two of the beam pockets on the west wall. The bolts, were set at a 5 3/4" gauge rather than the
required 2 3/4" gauge. We agreed that this could be solved by fabricating a steel beam bearing plate (3/8" thick)
with bolt holes to match the anchor bolts that have already been set. The bearing plate will be slipped over the
.anchor bolts; the anchor bolts will be cut off flush and plug welded to the plate. On top of the plate will be
welded 2-3/4" diameter threaded studs to replace the anchor bolts that should have been there. Jack will deal with
fabrication and installation of this plate. The anchor bolts will be set at the proper gauge in the east wall.
2. We discussed the need to leave an access hole in the masonry near the north entry way. Jack will tooth the block
around the comer and fill the sills with grout adjacent to this toothing.
3. I looked at the masonry work and bond beam reinforcing approximately two feet below the top of the west wall.
Bars are well tied and spaced with bar locators. Proper comer bars were in place. The vertical bars are properly
centered in the sells.
4.. Grouting of the west wall had occurred just before my arrival at the job site this morning, but everything appears
to be well done.
5. We discussed the bond beam reinforcing at control joints and agreed that the bars should be continuous through
these joints except where they are interrupted by the beam pockets.
