1, Michael S. Brake, a Colorado registered professional engineer, verify that the above referenced property in the City
of Fort Collins, Colorado, has been inspected by me or an appropriately qualified professional under my direct
supervision, and verify that construction of all stormwater facilities and grading is complete, and that the elevations
and drainage patterns shown on the attached drawing(s) which were developed from a survey performed on (Date):
July 11, 2014, accurately and truly depict the elevations and drainage pattems as they existed on the date of the
survey. Based on this information, I certify that (check one):
o The elevations and drainage pattems of the lottsite are in substantial conformance with the City approved
drainage and grading plan for the property (information attached). .
■ The elevations and drainage patterns of the lot/site as.measured are in general conformance with the City
approved drainage plan with exceptions (information attached). In my professional judgment the elevations
and grading of the lottsite as constructed are functionally adequate and equivalent to the specifically
approved drainage plan, and will affect stormwater doinagq�p d off 41cittsite in a manner consist

Glen,
The surveyor did shoot the main floor in front and back and that is indicated on the DC. The main floor
was raised to allow for a partially exposed basement, see attached photo. What we do not have is the
basement elevation if that needs to be added. The elevation on the north side of the house represents
the window well elevation as shown in the photo which I assume is your lowest opening into the
basement.
