Attached please find a completed Site Certification checklist, and accompanying
as -construction survey.
The site is very simple, in that we did not construct any storm sewer, inlets, detention
ponds, or spillways. The construction was limited to valley pans, curb and gutter, and
connections to existing streets.

Failure to properly disclose all known information could
result in the rejection of this certification and prevent the
release of any Stormwater Utility holds on Building Permits
and/or Certificates of Occupancy.
