complete, and the elevations and drainage patterns shown on the attached drawing(s) which were
developed from a survey performed on (Date) June 26, 2008 accurately and truly depict the
elevations and drainage patterns as they existed on the date of the survey. Based on this information,
certify that (check one):

approved drainage and grading plan for the property (information attached). SEE NOTE.
