complete, and the elevations and drainage patterns shown on the attached drawing(s) which were
developed from a survey, performed on (Date) June 26, 2008 , accurately and truly depict the
elevations and drainage patterns as they existed on the date of the survey. Based on this information, I
certify that (check one):

approved drainage and grading plan for the property (information attached). SEE NOTE.
