qualified professional under my direct supervision, and verify that construction of all stormwater facilities and
grading is complete, -and that the elevations and drainage patterns shown on the attached drawing(s) which were
developed from a survey performed on (Date): 08/05/15 , accurately and truly depict the elevations and
