FILE NO: 95-027
I am writing this letter to notify the City Stormwater Utility Department that the Shepherd of the
Hills Church has completed the required drainage improvements, per our report dated April 27,
1995. I have also enclosed the City's Drainage Certification Form and accompanying redlined
drawing as required for acceptance.
Our report directed the Church to construct a drainage swale to carry storm runoff from their new
addition south to the existing parking lot on the north side of the existing building. In addition, we
instructed the Church to install a walk -chase over the existing walk, to allow unobstructed passage
of storm runoff to the parking lot. Our erosion control facilities included the placement and
maintenance of hay -bale barriers, which they complied with.
As of my field inspection and survey on this day, I am confident the Church has constructed the
aforementioned items per our report and drainage plan. As the sod has not yet been replaced, as of
the time of this letter, we instructed the Church to maintain the hay bale barriers until such time as
the sediment loss from the site is no longer a threat. I am writing this letter to notify the City
Stormwater Utility Department of the Church's compliance with our recommendations.

the as -built conditions of the drainage facilities are as
described in the above form and in the redlined plans enclosed.
Failure to properly disclose all known information could
result in the rejection of this certification and prevent the
release of any Stormwater Utility holds on Building Permits
and/or Certificates of Occupancy.
