January 20, 2006 , accurately and truly depict the elevations and drainage patterns as they existed on
the date of the survey. Based on this information, I certify that (check one):
The elevations and drainage patterns of the lot are in substantial conformance with the City
approved drainage and grading plan or revised grading plan for the property (information
