King Surveyors visited the site on June 5, 2005, to confirm the installation of the storm pipe, structures,_ grading, and erosion control measures. Grading around the building provides positive drainage away -from the building and meets the intent of the proposed grading plan. The two swales east of the private drives have been installed and drain east and into the swale running along the east property line of the PVH Site. Riprap has been installed at the east ends of the private drives; in accordance with the proposed plans. It is important to note that sediment build-up has occurred in the swales during construction (see sheet CS26O).. The contractor is cleaning/regrading these areas in accordance with the erosion control plan for the site. Silt fence is not required along the east side of the Primary Care Plaza because grass is not yet established in the swale running south along the east property line, and erosion control is being provided by straw bales throughout the Swale (see sheet CS220 for details). Furthermore, silt fence was not installed along the south side of the Primary Cara Plaza site to allow construction equipment in and out of the site during construction of the parking lot. The contractor does not intend to put up the 473' of silt fence along the south boundary and 313' along the east boundary unless required. Therefore, if required, please have Bob Zakely contact Steve Cosgriff (Neenan Company) at 303-710-1902. to discuss if the fence is necessary and when the fence canbe removed. Stan$ec The two area inlets and corresponding storm drain system has been installed in the parking lot per the approved plans..It is important to note that the slope of the storm drain (Storm Q) from the 5' Type R inlet into storm manhole P-1 is 0.13%, rather than the approved 0.40%. Though this slope is sub -standard, the hydraulic gradeline for this storm line remains 4.0'+ below finished grade. Therefore, this slope should not pose any problems from a conveyance standpoint. Overall, the majority of elevations observed for storm drainage features.were within construction tolerances, except for Storm Q, as noted above. Therefore, the storm drain features and erosion control methods constructed and provided appear to be in compliance with approved plans for both the PVHS Third Filing (Primary Care Plaza), and also that of the PVHS Master Infrastructure Second Filing. This certification is signed and stamped. By signing this letter, I attest that to the best of my knowledge, the conditions of the site are as shown on the attached redlined photocopy. Failure to properly disclose all known information could result in the rejection of this certification and prevent the release of any storm water utility holds on the Building Permit or Certificates of Occupancy.
