of all stormwater facilities and grading there is complete, and that the elevations and drainage patterns
shown on the attached drawing(s) which were developed from a survey performed on (Date)
2-28-08 , accurately and truly depict the elevations and drainage patterns as they existed on the date of
the survey. Based on this information, I certify that (check one):

The elevations and drainage patterns of the lot as measured are in general conformance with the
