supervision, and verify construction of all stormwater facilities and grading is complete, and the
elevations and drainage patterns shown on the attached drawing was developed from a survey
performed on January xx, 2011, accurately and truly depict the elevations and drainage patterns as they
existed on the date of the survey. Based on this information, I certify that (check one):

approved drainage and grading plan for the property (information attached). SEE NOTE.
