of Fort Collins, Colorado, has been inspected by me or an appropriately qualified. professional under my direct
supervision, and verify that construction of all stormwater facilities and grading is complete, and that the elevations
and drainage patterns shown on the attached drawing(s) which were developed from a survey performed on October
30, 2010, accurately and truly depict the elevations and drainage patterns as they existed on the date of the survey.
Based on this information, I certify that (check one):
X The elevations and drainage patterns of the lot/site are in substantial conformance with the City approved
drainage and grading plan for the property (information attached).
• See attached excerpt from approved overlot grading plan for subdivision, along with as -built grading
plan for developed lot.

approved drainage plan with exceptions (information attached). In my professional judgment the
elevations and grading of the lot/site as constructed are functionally adequate and equivalent to the
