This is to verify that on October 12, 2005 the "As -Built" elevations of the lot grading,
building floors and lowest openings (top of window wells) of the newly constructed residence on
Lot 5 were surveyed under my supervision.
The grades and drainage patterns are in substantial compliance with approved grading
plan.
