elevations and drainage patterns shown on the attached drawing(s) which were developed from a survey
performed on (Date) April 13, 2007 , accurately and truly depict the elevations and drainage
patterns as they existed on the date of the survey. Based on this information, I certify that (check one):

approved drainage and grading plan for the property (information attached). SEE NOTE.
