the elevations and drainage patterns shown on the attached drawing(s) which were developed from a
survey performed on (Date) September 3. 2010 accurately and truly depict the elevations and
drainage patterns as they existed on the date of the survey. Based on this information, I certify that

approved drainage and grading plan for the property (information attached). SEE NOTE.
