is complete, and that the elevations and drainage patterns shown on the attached drawing(s) which were developed
from a survey performed on (Date): June 28, 2011 , accurately and truly depict the elevations and
drainage patterns as they existed on the date of the survey. Based on this information, I certify that (check one):

drainage and grading plan for the property (information attached). SEE NOTE.
