I, (print) , a Colorado registered professional engineer, verify that construction
of all stormwater facilities and grading is complete and hereby submit this certification.
Please answer the following questions by initialing the appropriate response. For each question initialed “No,” attach an
explanation detailing how this will affect stormwater drainage for this lot and any adjacent lots.
1.Certification includes attached Stormwater-approved grading plan. (NOTE: City’s
Building Department does not approve grading plans).
2.Certification includes the attached as-built survey dated (must be within 60
days of the date the certification is submitted) accurately and truthfully depicting
elevations for:
Top of foundation/finished floor
Finished grade on each side of the structure
Quarter points along each lot line (not more than 50 feet apart)
Break points/high points
Swales
Berms
Retaining walls
Air conditioning pads
Window wells
Any other appurtenance(s) within an easement
3.All elevations are within a 0.2 foot tolerance of the approved grading plan.
4.The top of foundation is at least 6 inches above the highest grade surrounding the
structure.
5.There is a minimum grade of 5% away from the structure within the first 5 to 10 feet
adjacent to the residence.
6.All slopes meet or exceed the minimum requirement of 2% (1% is acceptable on single
family lots when the swale is draining the runoff from only two adjoining properties)
and are not steeper than 33% (3:1).
7.All flow lines of stormwater drainage swales are a minimum of 6 inches below the
elevation of ground adjacent to the structure and will direct drainage as shown on the
Stormwater-approved grading plan.
8.All downspout locations and discharge directions are shown on the as-built survey.
Runoff is directed into an appropriate drainage system or lot line swale, and it is not
directed onto neighboring properties.

Note: The site in the immediate vicinity of the house/foundation, within 40± ft, is complete. Stockpiled material is to be used
for backfill when existing house is demolished and debris pile to be removed weather pending. Neither of these 2 items
affect the drainage performance around the building and do not adversly affect drainage onto neighboring properties.

opinion, the as-built survey is an accurate representation of the site conditions..

Note: The building location was adjusted in the field to accommodate the existing septic tank and
leach field. A grading exhibit was prepared and referenced for the final grades and is in general

EROSION CONTROL NOTES
1.CONTRACTOR TO PROVIDE AND MAINTAIN ALL NECESSARY EROSION CONTROL MEASURES PRIOR TO AND
THROUGHOUT CONSTRUCTION. PLACE EROSION CONTROL PROTECTION AT ALL EXISTING INLETS DOWNSTREAM
FROM THE CONSTRUCTION AREA. EROSION CONTROL PROTECTION SHALL REMAIN IN PLACE UNTIL CONSTRUCTION
IS COMPLETE AND ALL DISTURBED GROUND IS ADEQUATELY VEGETATED WITH APPROVAL FOR REMOVAL.
2.STRIP LIMITS OF STOCKPILE AREA, AS DELINEATED BY EARTHEN BERM, TO A DEPTH OF APPROXIMATELY 4" TO 6" TO
REMOVE ALL SURFACE VEGETATION, SOD, TREE STUMPS, AND ORGANIC TOPSOIL.
3.THE EXTENT OF EROSION CONTROL PROTECTION SHALL BE OBSERVED AND ADJUSTED AS NEEDED DURING
CONSTRUCTION BASED ON SITE CONDITIONS AND THE PERFORMANCE OF THE EROSION CONTROL MEASURES
SHOWN, TO SUPPLY SUFFICIENT EROSION CONTROL AND PREVENT OFFSITE SEDIMENTATION. ADDITIONAL MEASURES
AND MEANS NOT SHOWN SHALL ALSO BE IMPLEMENTED AS NEEDED. IT IS THE CONTRACTOR'S RESPONSIBILITY TO
DETERMINE APPROPRIATE BMPS DEPICTED AND/OR NOT DEPICTED ON THIS PLAN.
4.CONTRACTOR TO MINIMIZE DISTURBED AREA AT ALL TIMES AND LIMIT DISTURBANCE TO AREAS WITHIN EROSION
CONTROL BOUNDARIES. ADJUST EROSION CONTROL BOUNDARIES AS NECESSARY TO ADEQUATELY COMPLETE
PROPOSED WORK. AREAS SHALL NOT BE CLEARED UNTIL THEY ARE TO BE CONSTRUCTED UNLESS APPROVED BY THE

5.ALL DISTURBED AREAS SHALL BE PREPARED (SEEDED & MULCHED, SURFACE ROUGHENING, ETC.) TO MINIMIZE
EROSION POTENTIAL AND SOIL MIGRATION. ANY DISTURBED LAND MUST RECEIVE SURFACE ROUGHENING,
INCLUDING BUT NOT LIMITED TO SLOPED AREAS. ANY SOIL STOCKPILE REMAINING UNDISTURBED AFTER THIRTY (30)
DAYS SHALL BE SEEDED AND MULCHED.
UTILITY NOTES
1.ALL EXISTING UTILITY LOCATIONS SHOWN HAVE BEEN DETERMINED FROM A COMBINATION OF RECORD DRAWINGS,
SURVEYED FIELD MARK-OUTS AND PHYSICAL OBSERVATIONS PROVIDED BY OTHERS. THERE MAY BE OTHER UTILITIES
NOT SHOWN ON THE DRAWINGS WHICH PRESENTLY EXIST IN THE AREA OF CONSTRUCTION. THEREFORE, THE
CONTRACTOR SHALL VERIFY LOCATION AND DEPTH OF ALL EXISTING UTILITIES IN THE PROJECT VICINITY PRIOR TO
BEGINNING WORK. THE CONTRACTOR SHALL COORDINATE AND PROVIDE PROTECTION OF CONFLICTING UTILITIES.
NOTIFY OWNER/ENGINEER IF ANY CONFLICTS ARE ENCOUNTERED. THE OWNER AND/OR ENGINEER ASSUMES NO
RESPONSIBILITY FOR THE ACCURACY OR COMPLETENESS SHOWN.
2.ALL UTILITIES TO BE PROTECTED IN PLACE UNLESS OTHERWISE NOTED.
COFC STANDARD EROSION AND SEDIMENT CONTROL NOTES
1.THE CITY STORMWATER DEPARTMENT EROSION CONTROL INSPECTOR MUST BE NOTIFIED AT LEAST 24 HOURS PRIOR
TO ANY CONSTRUCTION ON THIS SITE.
2.ALL REQUIRED BMPS MUST BE INSTALLED PRIOR TO ANY LAND DISTURBING ACTIVITY (STOCKPILING, STRIPPING,
GRADING, ETC). ALL REQUIRED EROSION CONTROL MEASURES MUST BE INSTALLED AT THE APPROPRIATE TIME IN THE
CONSTRUCTION SEQUENCE AS INDICATED IN THE APPROVED PROJECT SCHEDULE, CONSTRUCTION PLANS, AND
EROSION CONTROL REPORT.
3.PRE-DISTURBANCE VEGETATION SHALL BE PROTECTED AND RETAINED WHEREVER POSSIBLE. REMOVAL OR
DISTURBANCE OF EXISTING VEGETATION SHALL BE LIMITED TO THE AREA REQUIRED FOR IMMEDIATE CONSTRUCTION
OPERATIONS, AND FOR THE SHORTEST PRACTICAL PERIOD OF TIME.
4.ALL SOILS EXPOSED DURING LAND DISTURBING ACTIVITY (STRIPPING, GRADING, UTILITY INSTALLATIONS,
STOCKPILING, FILLING, ETC.) SHALL BE KEPT IN A ROUGHENED CONDITION BY RIPPING OR DISKING ALONG LAND
CONTOURS UNTIL MULCH, VEGETATION, OR OTHER PERMANENT EROSION CONTROL IS INSTALLED. NO SOILS IN
AREAS OUTSIDE PROJECT STREET RIGHTS OF WAY SHALL REMAIN EXPOSED BY LAND DISTURBING ACTIVITY FOR MORE
THAN THIRTY (30) DAYS BEFORE REQUIRED TEMPORARY OR PERMANENT EROSION CONTROL (E.G. SEED/MULCH,
LANDSCAPING, ETC.) IS INSTALLED, UNLESS OTHERWISE APPROVED BY THE STORMWATER DEPARTMENT.
5.THE PROPERTY MUST BE WATERED AND MAINTAINED AT ALL TIMES DURING CONSTRUCTION ACTIVITIES SO AS TO
PREVENT WIND-CAUSED EROSION. ALL LAND DISTURBING ACTIVITIES SHALL BE IMMEDIATELY DISCONTINUED WHEN
FUGITIVE DUST IMPACTS ADJACENT PROPERTIES, AS DETERMINED BY THE CITY ENGINEERING DEPARTMENT.
6.ALL TEMPORARY (STRUCTURAL) EROSION CONTROL MEASURES MUST BE INSPECTED AND REPAIRED OR
RECONSTRUCTED AS NECESSARY AFTER EACH RUNOFF EVENT AND EVERY 14 DAYS IN ORDER TO ASSURE
CONTINUED PERFORMANCE OF THEIR INTENDED FUNCTION. ALL RETAINED SEDIMENTS, PARTICULARLY THOSE ON
PAVED ROADWAY SURFACES, SHALL BE REMOVED AND DISPOSED OF IN A MANNER AND LOCATION SO AS NOT TO
CAUSE THEIR RELEASE INTO ANY DRAINAGEWAY.
7.NO SOIL STOCKPILE SHALL EXCEED TEN (10) FEET IN HEIGHT. ALL SOIL STOCKPILES SHALL BE PROTECTED FROM
SEDIMENT TRANSPORT BY SURFACE ROUGHENING, WATERING, AND PERIMETER SILT FENCING. ANY SOIL STOCKPILE
REMAINING AFTER 30 DAYS SHALL BE SEEDED AND MULCHED.
8.CITY ORDINANCE PROHIBITS THE TRACKING, DROPPING, OR DEPOSITING OF SOILS OR ANY OTHER MATERIAL ONTO
CITY STREETS BY OR FROM ANY VEHICLE. ANY INADVERTENT DEPOSITED MATERIAL SHALL BE CLEANED IMMEDIATELY
BY THE CONTRACTOR.
