I, (print) , a Colorado registered professional engineer, verify that construction
of all stormwater facilities and grading is complete and hereby submit this certification.
Please answer the following questions by initialing the appropriate response. For each question initialed “No,” attach an
explanation detailing how this will affect stormwater drainage for this lot and any adjacent lots.
1.Certification includes attached Stormwater-approved grading plan. (NOTE: City’s
Building Department does not approve grading plans).
2.Certification includes the attached as-built survey dated (must be within 60
days of the date the certification is submitted) accurately and truthfully depicting
elevations for:
Top of foundation/finished floor
Finished grade on each side of the structure
Quarter points along each lot line (not more than 50 feet apart)
Break points/high points
Swales
Berms
Retaining walls
Air conditioning pads
Window wells
Any other appurtenance(s) within an easement
3.All elevations are within a 0.2 foot tolerance of the approved grading plan.
4.The top of foundation is at least 6 inches above the highest grade surrounding the
structure.
5.There is a minimum grade of 5% away from the structure within the first 5 to 10 feet
adjacent to the residence.
6.All slopes meet or exceed the minimum requirement of 2% (1% is acceptable on single
family lots when the swale is draining the runoff from only two adjoining properties)
and are not steeper than 33% (3:1).
7.All flow lines of stormwater drainage swales are a minimum of 6 inches below the
elevation of ground adjacent to the structure and will direct drainage as shown on the
Stormwater-approved grading plan.
8.All downspout locations and discharge directions are shown on the as-built survey.
Runoff is directed into an appropriate drainage system or lot line swale, and it is not
directed onto neighboring properties.

of the lot with portions of the pan at 0.6% slope
Survey did not include downspout locations. Arrows added based
on my on site observation. Side yard swales direct downspout
runoff either east or west and not to neighboring properties.

9.A smooth transition with no vertical offset (step) in grade is provided at the lot line to all
adjacent properties.

1.ALL EXISTING UTILITY LOCATIONS SHOWN HAVE BEEN PROVIDED BY OTHERS AND DETERMINED FROM A COMBINATION
OF RECORD DRAWINGS, SURVEYED FIELD MARK-OUTS AND PHYSICAL OBSERVATIONS. THERE MAY BE UTILITIES NOT
SHOWN ON THE DRAWINGS WHICH PRESENTLY EXIST IN THE AREA OF CONSTRUCTION. THEREFORE, THE CONTRACTOR
SHALL VERIFY LOCATION AND DEPTH OF ALL EXISTING UTILITIES IN THE PROJECT VICINITY PRIOR TO BEGINNING WORK.
THE CONTRACTOR SHALL BE FULLY AND SOLELY RESPONSIBLE FOR ANY AND ALL DAMAGES AND COSTS WHICH
MIGHT OCCUR BY THE CONTRACTOR'S FAILURE TO EXACTLY LOCATE AND PRESERVE ANY AND ALL UTILITIES. THE
CONTRACTOR SHALL NOTIFY ALL PUBLIC AND PRIVATE UTILITY COMPANIES AND DETERMINE THE LOCATION OF ALL
EXISTING UTILITIES PRIOR TO PROCEEDING WITH GRADING AND CONSTRUCTION. ALL WORK PERFORMED IN THE AREA
OF UTILITIES SHALL BE PERFORMED AND INSPECTED ACCORDING TO THE REQUIREMENTS OF THE UTILITY OWNER.
LIKEWISE, THE CONTRACTOR SHALL BE RESPONSIBLE FOR LOCATING AND MAPPING ANY EXISTING UTILITY (INCLUDING
DEPTH) WHICH MAY CONFLICT WITH THE PROPOSED CONSTRUCTION, AND FOR RELOCATING ENCOUNTERED UTILITIES
AS DIRECTED BY THE ENGINEER. CONTRACTOR SHALL CONTACT AND RECEIVE APPROVAL FROM OWNER/UTILITY
OWNER BEFORE RELOCATING ANY ENCOUNTERED UTILITIES. CONTRACTOR RESPONSIBLE FOR SERVICE
CONNECTIONS, AND RELOCATING AND RECONNECTING AFFECTED UTILITIES AS COORDINATED WITH UTILITY OWNER
AND/OR ENGINEER, INCLUDING NON-MUNICIPAL UTILITIES (TELEPHONE, GAS, CABLE, ETC.) WHICH SHALL BE
COORDINATED WITH THE UTILITY OWNER. THE CONTRACTOR SHALL IMMEDIATELY CONTACT ENGINEER UPON
DISCOVERY OF A UTILITY DISCREPANCY OR CONFLICT. AT LEAST 48 HOURS PRIOR TO CONSTRUCTION, THE
CONTRACTOR SHALL NOTIFY THE UTILITY NOTIFICATION CENTER OF COLORADO (1-800-922-1987, WWW.UNCC.ORG).
2.CONTRACTOR TO PROVIDE AND MAINTAIN ALL NECESSARY EROSION CONTROL MEASURES PRIOR TO AND
THROUGHOUT CONSTRUCTION. PLACE EROSION CONTROL PROTECTION AT ALL EXISTING INLETS DOWNSTREAM FROM
THE CONSTRUCTION AREA. EROSION CONTROL PROTECTION SHALL REMAIN IN PLACE UNTIL CONSTRUCTION IS
COMPLETE AND ALL DISTURBED GROUND IS ADEQUATELY VEGETATED WITH OWNER'S APPROVAL FOR REMOVAL.
3.THE EXTENT OF EROSION CONTROL PROTECTION SHALL BE OBSERVED AND ADJUSTED AS NEEDED DURING
CONSTRUCTION BASED ON SITE CONDITIONS AND THE PERFORMANCE OF THE EROSION CONTROL CONTROL
MEASURES SHOWN, TO SUPPLY SUFFICIENT EROSION CONTROL AND MINIMIZE DOWNSTREAM WATERWAY POLLUTION.
4.NO DEBRIS OR SEDIMENT SHALL BE TRACKED ONTO ALLEY OR PUBLIC STREET. IN CASE ANY TRACKING DOES OCCUR,
CONTRACTOR SHALL SWEEP AND CLEAN IMMEDIATELY.
5.LIMITS OF REMOVAL TO BE DETERMINED IN THE FIELD TO ADEQUATELY COMPLETE THE PROPOSED IMPROVEMENTS,
WHILE MAINTAINING, IF NOT IMPROVING, EXISTING CONDITIONS. ADDITIONAL REMOVAL IDENTIFIED IN THE FIELD
SHALL BE COMPLETED AT CONTRACTOR EXPENSE. ALL REPAIRS TO BE COMPLETED IN ACCORDANCE WITH CITY
STREET REPAIRS STANDARDS.
6.EXISTING CONCRETE AND ASPHALT PAVING SHALL BE SAWCUT AND REMOVED PRIOR TO EXCAVATION. REMOVE
CONCRETE TO THE NEAREST JOINT.
7.CONTRACTOR RESPONSIBLE FOR ALL SURFACE REPAIR/REPLACEMENT TO RESTORE SIDEWALKS, PAVEMENT,
CURB/GUTTER, AND SOD AREAS DISTURBED BY NEW WORK. REPAIR/REPLACEMENTS SHALL BE COMPLETED PER CITY
STANDARDS AT CONTRACTOR EXPENSE.
8.ALL PAVEMENTS AND EARTHWORK SHALL CONFORM TO THE GEOTECHNICAL REPORT FOR THE SITE. ALL SUBGRADE
TO BE PLACED/COMPACTED PER GEOTECHNICAL ENGINEER RECOMMENDATIONS.
9.REFERENCE ARCHITECTURAL AND STRUCTURAL PLANS FOR ADDITIONAL INFORMATION AND BUILDING DETAILS AND
DIMENSIONS. VERIFY ALL BUILDING DIMENSIONS PRIOR TO SITE WORK.
10.SIDEWALKS ADJACENT TO STREETS, DRIVES, PARKING LOTS, AND ACCESS SLOPE 2% TOWARDS THE CURB LINE.
11.PROVIDE A MINIMUM OF 5% SLOPE AWAY FROM FOUNDATIONS FOR THE FIRST 5 TO 10 FT.
12.THE FINISHED GRADE (FG) ADJACENT TO BUILDING FOUNDATIONS IS A MINIMUM 6 INCHES BELOW THE TOP OF
FOUNDATION (TF).
13.A FLOODPLAIN USE PERMIT IS REQUIRED PRIOR TO ANY CONSTRUCTION ACTIVITY, SUCH AS DRIVEWAYS, SIDEWALKS,
UTILITY INSTALLATION, ETC., IN THE FLOODPLAIN / FLOOD FRINGE.
14.ANY UTILITY WORK IN THE FLOODWAY MUST BE PRECEDED BY A FLOODPLAIN USE PERMIT AND NO-RISE
CERTIFICATION.
15.IN ADDITION TO MEETING APPLICABLE LCUASS, STATE OR FEDERAL STANDARDS, ALL MATERIALS, WORKMANSHIP AND
CONSTRUCTION OF PUBLIC WATER AND WASTEWATER SYSTEM IMPROVEMENTS SHALL MEET OR EXCEED THE
STANDARDS SET FORTH IN THE CITY OF FORT COLLINS WATER AND WASTEWATER DESIGN CRITERIA MANUAL.
16.IN CASES OF CONFLICT BETWEEN THESE SIGNED PLANS AND APPLICABLE STANDARDS, THE MOST RESTRICTIVE
STANDARD SHALL APPLY.
17.ALL WATER SERVICES SHALL BE TYPE K SOFT COPPER.
18.ALL SANITARY SEWER SERVICES SHALL BE SDR 35 PVC. VERIFY BUILDING CONNECTION ELEVATION (RE: MEP).
19.SITE CONTRACTOR SHALL COORDINATE UTILITY CONNECTIONS AT THE BUILDING WITH BUILDING CONTRACTOR.
20.COORDINATE ELECTRIC SERVICE ROUTING, DEMOLISHING, RELOCATING, AND MODIFICATIONS WITH THE ELECTRICAL

COFC STANDARD EROSION AND SEDIMENT CONTROL NOTES
1.THE CITY STORMWATER DEPARTMENT EROSION CONTROL INSPECTOR MUST BE NOTIFIED AT LEAST 24 HOURS PRIOR TO
ANY CONSTRUCTION ON THIS SITE.
2.ALL REQUIRED BMPS MUST BE INSTALLED PRIOR TO ANY LAND DISTURBING ACTIVITY (STOCKPILING, STRIPPING,
GRADING, ETC). ALL REQUIRED EROSION CONTROL MEASURES MUST BE INSTALLED AT THE APPROPRIATE TIME IN THE
CONSTRUCTION SEQUENCE AS INDICATED IN THE APPROVED PROJECT SCHEDULE, CONSTRUCTION PLANS, AND
EROSION CONTROL REPORT.
3.PRE-DISTURBANCE VEGETATION SHALL BE PROTECTED AND RETAINED WHEREVER POSSIBLE. REMOVAL OR
DISTURBANCE OF EXISTING VEGETATION SHALL BE LIMITED TO THE AREA REQUIRED FOR IMMEDIATE CONSTRUCTION
OPERATIONS, AND FOR THE SHORTEST PRACTICAL PERIOD OF TIME.
4.ALL SOILS EXPOSED DURING LAND DISTURBING ACTIVITY (STRIPPING, GRADING, UTILITY INSTALLATIONS, STOCKPILING,
FILLING, ETC.) SHALL BE KEPT IN A ROUGHENED CONDITION BY RIPPING OR DISKING ALONG LAND CONTOURS UNTIL
MULCH, VEGETATION, OR OTHER PERMANENT EROSION CONTROL IS INSTALLED. NO SOILS IN AREAS OUTSIDE PROJECT
STREET RIGHTS OF WAY SHALL REMAIN EXPOSED BY LAND DISTURBING ACTIVITY FOR MORE THAN THIRTY (30) DAYS
BEFORE REQUIRED TEMPORARY OR PERMANENT EROSION CONTROL (E.G. SEED/MULCH, LANDSCAPING, ETC.) IS
INSTALLED, UNLESS OTHERWISE APPROVED BY THE STORMWATER DEPARTMENT.
5.ALL TEMPORARY (STRUCTURAL) EROSION CONTROL MEASURES MUST BE INSPECTED AND REPAIRED OR
RECONSTRUCTED AS NECESSARY AFTER EACH RUNOFF EVENT AND EVERY 14 DAYS IN ORDER TO ASSURE CONTINUED
PERFORMANCE OF THEIR INTENDED FUNCTION. ALL RETAINED SEDIMENTS, PARTICULARLY THOSE ON PAVED
ROADWAY SURFACES, SHALL BE REMOVED AND DISPOSED OF IN A MANNER AND LOCATION SO AS NOT TO CAUSE
THEIR RELEASE INTO ANY DRAINAGEWAY.
6.CITY ORDINANCE PROHIBITS THE TRACKING, DROPPING, OR DEPOSITING OF SOILS OR ANY OTHER MATERIAL ONTO
CITY STREETS BY OR FROM ANY VEHICLE. ANY INADVERTENT DEPOSITED MATERIAL SHALL BE CLEANED IMMEDIATELY BY
THE CONTRACTOR.
