my direct supervision, and verify construction of all storm water facilities and grading is complete, and
the elevations and drainage patterns shown on the attached drawing(s) which were developed from a
survey performed on (Date) February 4, 2009 , accurately and truly depict the elevations and
drainage patterns as they existed on the date of the survey. Based on this information, I certify that

□ The elevations and drainage patterns of the lot/site are in substantial conformance with the City
approved drainage and grading plan for the property (information attached). SEE NOTE.
■ The elevations and drainage patterns of the lot/site as measured are in general conformance with
